达成型品管圈活动在ICU危重症患者中的护理效果分析

摘　　要：目的:探讨课题达成型品管圈(QCC)活动在ICU危重症患者中的护理效果。方法:选取2019年9月—2020年5月入住我院ICU的危重症患者150例。将实施达成型品管圈活动前的75例患者设为对照组,将实施达成型品管圈活动后的75例患者设为观察组,比较两组护理效果。成立品管圈小组,确定“构建ICU医护联合VTE标准化预防体系”活动主题,根据达成型品管圈实施步骤,按照主题选定、课题明确化、目标设定、方策拟定、最佳方策追究、方策实施与检讨、效果确认的流程开展活动。结果:对照组静脉血栓栓塞症(VTE)预防措施实施率对照组为44.2%,观察组为100.0%;VTE入室筛查率对照组为59.9%,观察组为100.0%;出血症状观察率对照组为81.0%,观察组为96.6%;气压治疗实施率对照组为47.7%,观察组为100.0%;医生VTE预防知识掌握率由58.1%提高至83.9%;护士VTE预防知识掌握率由54.1%提高至80.6%。以上指标两组比较,差异有统计学意义(P<0.05)。对全体圈员在品管圈活动实施前后的综合能力进行评分,活动实施后圈员在各方面(解决问题的能力、责任心、沟通协调能力、自信心、团队精神、积极性、品管手法、和谐度)的能力均有所提升。结论:达成型品管圈活动在ICU危重症护理中效果显著,值得应用。Objective To explore the role of project achievement quality control circle(QCC)in improving the implementation of preventive measures for venous thromboembolism(VTE)in severe patients.Methods Patients admitted to ICU of our hospital from September 2019 to May 2020 were selected.According to the implementation of achievement type quality control circle,it is divided into pre QCC group and post QCC group,with 75 patients in each group.Establish a quality control circle group to determine the activity theme of“building a standardized prevention system of ICU medical care joint VTE”,and carry out activities according to the implementation steps of the project achievement quality control circle and the process of theme selection,project clarification,goal setting,formulation of policies,investigation of the best policies,implementation and review of policies and confirmation of effects.Results After the activity,the implementation rate of VTE prevention measures increased from 44.2%to 100%,the VTE in-house screening rate increased from 59.9%to 100%,the observation rate of bleeding symptoms increased from 81%to 96.6%,the implementation rate of barometric therapy increased from 47.7%to 100%,and the Mastery rate of doctors’VTE prevention knowledge increased from 58.06%to 83.85%.The mastery rate of VTE prevention knowledge among nurses increased from 54.07%to 80.64%.Score the comprehensive ability of all the circle members before and after the implementation of the quality control circle activity.After the implementation of the activity,the ability of the circle members in all aspects(problem-solving ability,sense of responsibility,communication and coordination ability,self-confidence,team spirit,enthusiasm,quality control methods and harmony)has been improved.Conclusion The application of project achievement quality control circle to construct the joint standardized VTE prevention system of ICU medical care can improve the implementation rate of VTE prevention measures of ICU nurses,which is recommended to be wid
