电针联合耳穴压豆配合三阶梯药物治疗癌痛的疗效观察

摘　　要：目的探讨电针联合耳穴压豆配合三阶梯药物治疗癌痛的临床疗效。方法选取2019年3月至2020年3月本院肿瘤科收治的癌痛患者60例,按照随机数字表法分为观察组与对照组,每组各30例。观察组采用电针联合耳穴压豆配合WHO三阶梯药物治疗,对照组单用WHO三阶梯药物治疗。连续治疗14天后,比较两组患者疼痛程度(NRS评分)、疼痛缓解率、吗啡用量、生活质量(KPS评分)及不良反应发生率。结果治疗14天后,两组患者的疼痛NRS评分均低于治疗前,且观察组较对照组下降明显,差异有统计学意义(P<0.05);观察组的疼痛缓解率(80.0%)优于对照组(60.0%),差异有统计学意义(P<0.05);观察组的吗啡用量较治疗前减少,差异有统计学意义(P<0.01);对照组的吗啡用量较治疗前增加,差异有统计学意义(P<0.01);两组的KPS评分均较治疗前升高,且观察组的KPS评分优于对照组,差异有统计学意义(P<0.01)。观察组出现恶心、呕吐、头晕、便秘等不良反应的发生率显著低于对照组,差异有统计学意义(P<0.05)。结论电针联合耳穴压豆配合三阶梯药物治疗癌痛的疗效确切,还可以减少中重度癌痛患者的吗啡用量、改善患者生存质量、降低不良反应发生率。Objective To observe the clinical effect of electroacupuncture and auricular point pressing combined with three-step analgesic ladder on relief of cancer pain.Methods A total of 60 cancer pain patients admitted to the Department of Oncology of our hospital from March 2019 to March 2020 were selected and divided into observation group and control group according to random number table method,with 30 patients in each group.The observation group was treated by electroacupuncture and auricular point pressing combined with WHO three-step analgesic ladder for cancer pain management,and the control group was treated with WHO three-step analgesic ladder only.After treatment for 14 days,pain degree(NRS score),pain relief rate,morphine dosage,quality of life(KPS score)and incidence of adverse reactions were compared between two groups.Results After 14 days of treatment,the NRS score of pain in two groups was lower than before,the observation group decreased significantly than the control group,and the difference was statistically significant(P<0.05).The pain relief rate of the observation group(80.0%)was better than that of the control group(60.0%),and the difference was statistically significant(P<0.05).The dosage of morphine in the observation group was significantly lower than that before treatment(P<0.01).The dosage of morphine in the control group increased compared with that before treatment,and the difference was statistically significant(P<0.01).The KPS score of both groups was higher than that before treatment,and the KPS score of observation group was better than that of control group,and the difference was statistically significant(P<0.01).The incidence of nausea,vomiting,dizziness,constipation and other adverse reactions in the observation group was significantly lower than that in the control group,with statistical significance(P<0.05).Conclusion The combination of electroacupuncture and vauricular point pressing combined with three-step analgesic ladder can effectively relieve the cancer pain,and it can also
