血栓弹力图常用参数与凝血因子及凝血象间的相关性

摘　　要：目的研究血栓弹力图(TEG)常用参数与凝血因子及凝血象间的相关性。方法选择2018年2月-2020年2月在我院收治的需输血治疗的患者共123例。所有患者均采用TEG和常规凝血功能试验进行检查。pearman相关性分析TEG常用参数分别于与凝血因子及凝血象间的相关性;多元回归方程分析TEG常用参数与凝血因子及凝血象间的相关性。结果Pearman相关性分析凝血因子中凝血因子Ⅰ(FIB)与TEG常用参数R时间无相关性(P>0.01),与K时间、Angle夹角、Ma、G值、综合凝血指数(Cl)具有较高的相关性(P<0.01);凝血因子Ⅷ与凝血因子Ⅸ与TEG常用所有参数均具有较高的相关性(P<0.01);其余凝血因子与TEG常用参数无相关性(P>0.01)。凝血象中APTT与R时间、K时间、Angle夹角、Cl具有较高的相关性(r=0.553,r=0.259,r=-0.258,r=-0.398,P<0.01);TT与Ma、G值具有较高的相关性,(r=-0.341,r=-0.321,P<0.01);PT与TEG常用参数均无相关性(P>0.01)。多元回归方程分析仅有Fig和APTT与6个常用参数之间存在回归关系(P<0.01)。结论TEG常用参数与FIB、凝血因子Ⅷ与凝血因子Ⅸ相关性较高,与凝血象中APTT、TT间的相关性较高。Objective To investigate the correlation between the common parameters of thromboelastography(TEG)and coagulation factors and coagulation image.Methods A total of 123 patients with blood transfusion treatment in our hospital from February 2018 to February 2020 were selected.All patients were examined by TEG and routine coagulation function test before treatment.Spearman correlation analysis was used to analyze the correlation of TEG parameters with coagulation factors and coagulation images;Multiple regression equation was used to analyze the correlation of TEG parameters with coagulation factors and coagulation images.Results Spearman correlation analysis of coagulation factor coagulation factor fibrinogen(FIB)showed no correlation with TEG common parameter R time(P>0.01),while had close correlation with K time,α-Angle,Ma,G value,and coagulation index(CI),with statistic difference(P<0.01);Coagulation factorⅧand coagulation factorⅨyielded a high correlation with all common parameters of TEG(P<0.01),and the rest coagulation factors showed no correlation with the TEG parameters(P>0.01).APTT had a close correlation with R time,K time,α-Angle,CI(r=0.553,r=0.259,r=-0.258,r=-0.398,P<0.01);TT was significantly correlated with Ma and G values(r=-0.341,r=-0.321,P<0.01);PT showed no significant correlation with all those TEG parameters(P>0.01).Multiple regression equation analysis showed that there was a regression relationship between Fig and APTT and six common parameters(P<0.01).Conclusion The common parameters of TEG are highly correlated with FIB,coagulation factor VIII and coagulation factor IX,APTT and TT.
