摘  要:糖尿病肾病(DN)是糖尿病最为严重的并发症之一,也是终末期肾病的重要病因,氧化应激是DN发生发展、肾功能损伤的主要致病机制。查阅国内外相关文献,从氧化应激机制和海洋药物干预治疗2个角度进行归纳总结,综述了多元醇、AGEs、PKC、己糖胺通路,NOX、Nrf2/HO-1、AMPK因子,SGLT2、HIF-1α、肠道菌群靶点和炎症、纤维化病理损伤。实验和临床研究表明复方、多糖、小分子等海洋药物对DN具有较好的防治作用,属中医药治疗思路和方法,具有多靶点和整体调节的优势。详细阐述海洋药物防治DN药效物质基础和作用机制,可为海洋药用成分的开发应用提供新的研究思路。Diabetic nephropathy(DN) is one of the most serious complications of diabetes and an important cause of end-stage renal disease. Oxidative stress is the main pathogenic mechanism of the occurrence and development of DN and renal function damage. Related literatures were reviewed and summarized from the perspectives of oxidative stress mechanism and marine drug intervention. Polyol, AGEs, PKC, hexosamine pathway, NOX, Nrf2/HO-1, AMPK factor, SGLT2, HIF-1α, gut microbiota, and pathological injury of inflammation and fibrosis were summarized. Experimental and clinical studies showed that marine drugs such as compound, polysaccharide, and micromolecule had good prevention and control effects on DN, which belonged to the treatment ideas and methods of traditional Chinese medicine, and had advantages of multitarget and overall regulation. The detailed elaboration of the pharmacodynamic basis and action mechanism of marine drugs against DN could provide new research ideas for the development and application of marine medicinal ingredients.
