富血小板纤维蛋白治疗慢性伤口的临床研究

摘　　要：目的采用自身对照研究评价富血小板纤维蛋白临床治疗慢性伤口的效果。方法以无抗凝剂的无菌真空试管采集患者自体静脉血10 ml或20 ml,放入离心机内以3000 r/min速度进行对角离心12 min;取出试管中间凝胶状初级产物,挤压成膜。平摊于伤口上,用透明膜进行固定,1周后揭除透明膜清创换药,记录伤口性状、面积或体积,10 d为1个治疗周期。选择以此方法治疗的慢性伤口患者12例,比较治疗前后伤口床征评分、伤口面积、治疗时间及治疗费用等指标变化。统计数据采用Wilcoxon符号秩检验。结果12例慢性伤口患者伤口床征评分治疗前为(3.25±1.91)分,治疗后为(14.17±1.90)分(Z=3.086,P<0.01),创面平均面积治疗前为(16.84±13.27)cm^(2),治疗后创面平均面积缩小至(3.01±3.84)cm^(2)(Z=-3.059,P<0.01),平均缩小(81.84±19)%,治疗前已治疗时间为(67.25±30.54)d;治疗后患者伤口痊愈或接近痊愈的时间为(23.33±11.51)d(Z=-2.981,P<0.01),治疗前平均花费为(4525.83±3224.72)元,显著高于富血小板纤维蛋白治疗期间的平均费用[(1110.83±367.61)元,Z=-3.059,P<0.01],差异均有统计学意义(P<0.01)。结论富血小板纤维蛋白对于促进慢性伤口愈合有显著效果,具有很高的实用推广价值。Objective To investigate the effectiveness of clinical application of Choukroun’platelet-rich fibrin(PRF)on chronic wounds by self control research.Methods Venous blood of 10 ml or 20 ml from the patients was drawn into a sterile vacuum glass tube without anticoagulant,and immediately centrifuged at 3000 r/min for 12 min.A fibrin clot containing the platelets in the middle of sample was pulled out.After squeezed into membrane,PRF was applied onto the wound and covered with a transparent plastic membrane.After one week,the plastic membrane was uncovered,the wound was processed with debridement,then the feature,the proportion or the volume of the wound was measured.A treatment period was 10 days.A total of 12 patients with chronic wounds treated with PRF therapy were enrolled in the research.Wound bed score(WBS),the wound area,duration of treatment and the expense were compared before and after the treatment.The Wilcoxon rank-sum test was used for difference test.P<0.01 was considered as significant.Results Before PRF treatment,the average WBS score of all 12 patients was(3.25±1.91,Z=3.086,P<0.01),and after the treatment it was(14.17±1.90).The average wound area before and after PRF was(16.84±13.27)cm^(2) and(3.01±3.84)cm^(2) respectively(Z=-3.059,P<0.01).Duration of healing or improving was(23.33±11.51)days after the PRF treatment,and treatment time was(67.25±30.54)days before the PRF treatment(Z=-2.981,P<0.01).The expense of the PRF treatment was(1110.83±367.61)Yuan RMB,and that berore the PRF treatment was(4525.83±3224.72)Yuan RMB(Z=-3.059,P<0.01).Conclusion PRF facilitates the healing of chronic wounds effectively.
