摘　　要：胰腺癌恶性程度高,预后差。目前根治性手术切除是延长生存期的有效手段,然而可直接进行根治性切除的胰腺癌病人不足20%。新辅助治疗可通过缩小肿瘤体积,改善肿瘤与邻近血管的关系,以争取手术机会,并提高手术切除率及R0切除率,从而延长生存期,但如何准确有效地评估新辅助治疗疗效尚不明确。目前临床上常用CA19-9及增强CT等,一定程度上可评价新辅助治疗的疗效,但存在一定的局限性。其他肿瘤标记物及新兴生物标记物的探索、PET和MRI等影像学指标的深入研究、营养指标的应用为新辅助治疗疗效评估提供了新的视角。随着相关领域逐步深入的研究及临床验证,有望能精准评估胰腺癌新辅助治疗疗效,从而优化胰腺癌病人的综合诊疗策略。Pancreatic cancer has a poor prognosis for its high malignancy.Radical surgical resection is an effective means for prolonging survival.However,less than 20%of patients can be directly treated with surgical resection.At present,Neoadjuvant therapy can reduce the tumor size,improve the relationship between the tumor and adjacent blood vessels,to gain surgical opportunities and increase the rate of surgical resection,thereby prolonging survival.However,the evaluation strategy of neoadjuvant therapy response remains controversial.At present,CA19-9 and enhanced CT are commonly used in clinical practice,which can evaluate the efficacy of neoadjuvant therapy to a certain extent,but there are still some shortcomings.The exploration of other tumor markers and emerging biomarkers,the in-depth research of imaging such as PET and MRI,and the application of nutritional indicators have provided new perspectives for evaluating the efficacy of neoadjuvant therapy.With the gradual in-depth research and clinical verification in related fields,it is expected to accurately assess the effectiveness of neoadjuvant therapy for pancreatic cancer,thereby optimizing the comprehensive diagnosis and treatment strategy for patients with pancreatic cancer.
