摘　　要：目的 探究肺组织纹理超声图像定量分析用于预测生长受限胎儿肺成熟度的价值,为临床开展无创性评估胎儿肺成熟度提供科学依据。方法 本研究于2019年1月至2021年6月在我院选择55名伴有胎儿生长受限孕妇纳入观察组,并选择55名健康孕妇作为对照。通过超声检查获取胎儿肺部图像,采用QuantusFLM软件进行肺组织纹理定量分析,评估胎儿肺成熟度。结果 在纳入分娩孕周为34~39周的110名胎儿中,肺组织纹理超声图像定量分析肺成熟预测效果指标分别如下:灵敏度(96.2%)、特异度(25.0%,1/4)、阳性预测值(97.1%)、阴性预测值(20.0%,1/5)。观察组和对照组胎儿肺成熟预测灵敏度分别为96.2%、96.2%,阳性预测值分别为98.1%和96.2%。结论 采用肺组织纹理超声图像定量分析预测生长受限胎儿肺成熟度具有良好的灵敏度和阳性预测值,在临床实践中应用价值较大。Objective To explore the value of quantitative analysis of lung tissue texture ultrasound images in predicting fetal lung maturity with growth restriction, and to provide scientific basis for clinical non-invasive assessment of fetal lung maturity. Methods 55 pregnant women with fetal growth restriction were selected as the observation group, and 55healthy pregnant women were selected as the control group. Fetal lung images were obtained by ultrasound examination, and quantitative analysis of lung tissue texture was carried out by QuantusFLM software to evaluate fetal lung maturity. Results The indicators of lung maturity prediction by quantitative analysis of lung tissue texture ultrasound images are as follows:sensitivity(96.2%), specificity(25.0%, 1/4), positive predictive value(97.1%) and negative predictive value(20.0%, 1/5). The sensitivity and positive predictive value of fetal lung maturity in observation group and control group were 96.2% and 96.2%respectively, and 98.1% and 96.2% respectively. Conclusion Quantitative analysis of lung tissue texture ultrasound images to predict the lung maturity of growth-limited fetus has good sensitivity and positive predictive value, and it has great application value in clinical practice.
