上焦宣痹汤治疗肺失宣降型梅核气经验探析

摘　　要：梅核气是一种咽喉部有紧绷感或异物感的功能性疾病。该病多为肝气郁结、痰气交阻于喉间所致,一般无痛感,不伴随吞咽功能障碍,但病程较长,易反复发作。基于圆运动学说的气机升降斡旋理论,就肺失宣降型梅核气进行辨证论治,肺金右降失施,则肝木左升无源,进而四维运转不利,影响中气斡旋。运用上焦宣痹汤清宣肺痹,佐以疏肝理气之药以助肺气敛降,运轮复轴。同时配伍健运中轴脾胃之药,运轴行轮。轴轮同调,从而促进人体气机周流运转。以此辨治梅核气疗效显著,并附验案一则,以期为临床治疗梅核气提供参考。Globus hystericus is a subjective sensory disease with tension or foreign body sensation in the throat.The disease is mostly caused by stagnation of liver-qi and obstruction of phlegm-qi between the throat,generally painless,not accompanied by swallowing dysfunction,but the course of the disease is longer,easy to attack repeatedly.Based on the qi rising and lowering mediation theory of circular movement theory,the author makes a treatment based on syndrome differentiation of globus hystericus of“impaired depurative descending of lung qi”tape.If the lung qi does not fall smoothly from the right,then there is no driving force for the liver qi to rise from the left,and then the four-dimensional operation is unfavorable,affecting the mediation of middle energizer.The author uses Shangjiao Xuanbi decoction to clear lung obstruction,combined with drugs for dispersing liver and regulating qi to help lung qi converge and descend,driving the wheel and thus transporting the axis.At the same time,it is compatible with the medicine that operate the spleen and stomach of middle energizer,transporting the axis and thus driving the wheel.The axis and wheel are adjusted at the same time to promote the circumfluence of the body’s qi.Based on this,the curative effect of differentiation and treatment of globus hystericus is remarkable,and one case is attached to provide ideas for clinical treatment of globus hystericus.
