摘 要:目的:观察唑来膦酸注射液联合维生素D配伍碳酸钙治疗骨质疏松患者的效果。方法:选取116例骨质疏松患者为研究对象,按照数字随机数字表法分为对照组和观察组各58例。对照组给予碳酸钙配伍维生素D治疗,观察组在对照组基础上联合唑来膦酸注射液治疗,比较两组临床疗效,治疗前后血清骨转换标志物水平、骨密度、腰椎功能[腰椎Oswestry功能障碍指数(ODI)]评分及不良反应发生率。结果:观察组治疗总有效率为89.66%,高于对照组的75.86%,差异有统计学意义(P<0.05);治疗后,观察组Ⅰ型前胶原氨基端前肽、Ⅰ型胶原β降解产物水平均低于对照组,25羟维生素D水平高于对照组,差异有统计学意义(P<0.05);治疗后,观察组股骨颈、髋关节、L1~4节段等骨密度水平均高于对照组,差异有统计学意义(P<0.05);治疗后,观察组ODI评分低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:唑来膦酸注射液联合维生素D配伍碳酸钙治疗骨质疏松患者,可提高临床疗效,改善血清骨转换标志物水平,提高骨密度,促进关节功能恢复,其效果优于单纯碳酸钙配伍维生素D治疗。Objective: To observe effects of Zoledronic acid injection combined with Vitamin D and Calcium carbonate in treatment of patients with osteoporosis. Methods: 116 patients with osteoporosis were selected as the research objects and were divided into control group and observation group according to the random number table method, 58 cases in each group. The control group was treated with Calcium carbonate combined with Vitamin D, while the observation group was treated with Zoledronic acid injection on the basis of those of the control group. The clinical efficacy, the serum bone turnover marker levels, the bone mineral density, the lumbar spine function [lumbar Oswestry disability ondex(ODI)]score and incidence of adverse reactions were compared between the two groups before and after the treatment. Results: The total effective rate of the observation group was 89.66%, which was higher than 75.86% of the control group, and the difference was statistically significant(P<0.05). After the treatment, the levels of procollagen type I N-terminal propeptide and type I collagen β degradation products in the observation group were lower than those in the control group;the level of 25-hydroxyvitamin was higher than that in the control group;and the difference was statistically significant(P<0.05). After the treatment, the BMD levels of the femoral neck, hip joint and L1-4 segments in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05). After the treatment, the ODI score of the observation group was lower than that of the control group, and the difference was statistically significant(P<0.05). However, there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).
