摘　　要：目的评估256层多螺旋CT小肠造影(multi-slice spiral computed tomography enteroclysis,MSCTE)在诊断小肠结核中的价值。方法选取我院2015年1月~2020年12月63例疑似小肠结核患者行256-MSCTE检查,阳性病例与病理结果相对照,阴性病例与临床随访结果相对照,随访时间6~9个月,平均7.5个月。采用统计学软件SPSS 25.0计算256-MSCTE诊断小肠结核的效能,以评估其诊断小肠结核的价值。结果63例受检者中,61例耐受256-MSCTE检查;2例受检者因出现反复呕吐而不耐受检查。256-MSCTE诊断阳性病例45例,阴性病例18例;最终确诊阳性病例46例,阴性病例17例。256-MSCTE诊断小肠结核的敏感度为93.48%、特异度为88.24%、阳性预测值为95.56%、阴性预测值为83.33%、诊断符合率为92.06%、误诊率为11.76%、漏诊率为6.52%、约登指数为81.71%。结论256-MSCTE诊断小肠结核的效能较高,是评价和诊断小肠结核实用且准确的检查方法,具有较高临床价值。Objective To evaluate the value of 256-MSCTE in the diagnosis of small intestinal tuberculosis.Methods From January 2015 to December 2020,63 patients with suspected small intestinal tuberculosis in our hospital were selected for 256-MSCTE examination.The positive cases were compared with the pathological results,and the negative cases were compared with the clinical follow-up results(the follow-up time was 6-9 months,with an average of 7.5 months).SPSS 25.0 was used to calculate the effectiveness of 256-MSCTE in the diagnosis of small intestinal tuberculosis,so as to evaluate the value of 256-MSCTE in the diagnosis of small intestinal tuberculosis.Results Among 63 subjects,61 subjects tolerated 256-MSCTE;2 subjects did not tolerate 256-MSCTE because of repeated vomiting.There were 45 positive cases and 18 negative cases diagnosed by 256-MSCTE;46 positive cases and 17 negative cases were finally confirmed.The sensitivity was 93.48%,the specificity was 88.24%,the positive predictive value was 95.56%,the negative predictive value was 83.33%,the diagnosis coincidence rate was 92.06%,the misdiagnosis rate was 11.76%,the missed diagnosis rate was 6.52%and the Youden index was 81.71%.Conclusion 256-MSCTE has high efficiency in the diagnosis of small intestinal tuberculosis.It is a practical and accurate examination method for the evaluation and diagnosis of small intestinal tuberculosis,and has high clinical value.
