摘  要:目的:观察并确定慢性刺激对糖尿病大鼠血清TNF-α和IL-18水平的影响,探讨糖尿病并发抑郁症的病理机制。方法:将30只清洁级雄性SD大鼠随机分为3组:正常对照组(Control组)10只,糖尿病模型组(DM组)10只,糖尿病合并抑郁症组(DM+UCMS组)10只。应用尾静脉注射链脲佐菌素(STZ)联合高脂高糖饮食诱导2型糖尿病大鼠模型;在DM模型的基础上增加慢性刺激诱导DM+UCMS动物模型。观察各组大鼠一般情况的变化,检测体质量、血糖、血清胰岛素、血清TNF-α和IL-18等指标,采用强迫游泳实验检测大鼠行为学变化。结果:①随着各种刺激的不断进行,与对照组大鼠比较,DM与DM+UCMS大鼠体质量均显著降低(P<0.001),其中DM+UCMS组大鼠的体质量最低。②与实验前相比,DM组和DM+UCMS组大鼠强迫游泳的挣扎时间和游泳明显减少(P<0.001),漂浮的时间明显延长(P<0.05和P<0.001);其中DM+UCMS组大鼠游泳时间减少最为显著(P<0.001)。③与对照组相比较,DM组、DM+UCMS组空腹血糖及胰岛素水平均显著升高(P<0.001),胰岛素抵抗指数明显增加(P<0.001)。④与对照组相比,DM和DM+UCMS组大鼠的血清TNF-α和IL-18水平明显升高(P<0.001和P<0.05);与DM组相比,DM+UCMS组大鼠血清TNF-α和IL-18水平显著增加(P<0.05和P<0.001)。结论:糖尿病合并抑郁症大鼠血清TNF-α和IL-18表达水平升高,炎症反应增强是糖尿病并发抑郁症的重要病理机制之一。Objective:This study observes and determines the effects of chronic mild stress on the levels of serum TNF-α and IL-18 in rats with diabetes,and explore the pathological mechanism of diabetes complicated with depression.Method:Thirty SPF male SD rats were randomly divided into three groups:control group,diabetes model group(DM),and diabetes combined with depression group(DM+UCMS group).There are 10 rats in each group.The rat model of type 2 diabetes was induced by tail vein injection of streptozotocin(STZ)combined with a high-fat and high-sugar diet.On the basis of the DM model,unpredictable chronic mild stress was added to induce the DM+UCMS animal model.Observe the changes in the general conditions of the rats in each group,detect the weight,serum glucose,serum insulin,serum TNF-α and IL-18,and the forced-swimming test was employed to detect the behavioral changes of the rats.Results:①With the progress,the body weight of DM and DM+UCMS rats was significantly reduced compared with the control group(P<0.001),and the weight of rats in the DM+UCMS group was the lowest.②Compared with before the experiment,the climbing time of forced-swimming test in the DM group and DM+UCMS group were significantly reduced(P<0.001),and the immobile time was significantly prolonged in DM and DM+UCMS group(P<0.05 and P<0.001).The immobility time of rats in the DM+UCMS group increased the most significantly(P<0.001).③Compared with the control group,fasting serum glucose and insulin levels in the DM group and DM+UCMS group were significantly increased(P<0.001),and the insulin resistance index was significantly increased(P<0.001).④Compared with the control group,the levels of serum TNF-α and IL-18 in the DM and DM+UCMS groups were significantly increased(P<0.001 and P<0.05).Compared with the DM group,the serum TNF-α and IL-18 levels in the DM+UCMS group increased significantly(P<0.05 and P<0.001).Conclusion:Chronic mild stress could lead to increased serum TNF-αand IL-18 in rats with diabetes,and enhanced inflammation is one
