摘  要:目的调查临床医生作为第二受害者在经历医疗不良事件后的经历及支持需求现状。方法于2019年6月—2020年6月采用方便抽样法选取中山市4家三级甲等医院在临床工作中经历医疗不良事件的医生作为调查对象,采用一般资料问卷和第二受害者经历及支持量表进行调查。结果医生作为第二受害者在性别、婚姻状况、文化程度方面的痛苦得分比较无统计学意义(P>0.05);在年龄、工作年限、工作科室以及职称方面的痛苦得分组间比较有统计学意义(P<0.05),其中年龄>40 y、工作年限>20 a、副主任/主任医师高级职称以及儿科专业的第二受害者心理和生理痛苦得分最高。在组织支持方面,第二受害者在性别、婚姻方面无统计学意义(P>0.05);在年龄、学历、职称、工作年限和专业方面有统计学意义(P<0.05);离职/转岗方面在性别、婚姻、工作年限以及专业方面有统计学意义(P<0.05)。结论第二受害者在医疗不良事件后出现巨大的痛苦经历,得到的组织支持不足,易产生职业倦怠感,出现离职或离岗现象。因此,组织机构应该加强对第二受害者的关注力度,提供强大的组织支持系统,减轻第二受害者的痛苦经历,降低离职率,保证医疗质量安全和满意度。Objective To investigate the experience and support needs of clinicians as secondary victims of adverse medical events.Methods From June 2019 to June 2020,convenience sampling was used to select doctors who experienced adverse medical events in 4 tertiary hospitals in Zhongshan city as respondents.General questionnaire and secondary victim experience and support scale were used for the investigation.Results There was no statistical significance in the secondary victims’pain scores in gender,marital status and education level(P>0.05).There was statistical significance in age,year of working,departments and professional titles(P<0.05),among which the secondary victims above 40 years,working for more than 20 years,with senior professional titles of associate directorship or chief physician/surgeon,and in pediatric department had the highest pain scores both psychologically and physically.In terms of organizational support,gender and marriage of the secondary victims showed no statistical significance(P>0.05).There was statistical significance in age,education background,professional title,working years and major(P<0.05).Among those who resigned and transferred to other posts,the differences were statistically significant in gender,marriage,working year and major(P<0.05).Conclusion The secondary victims experience great pain after adverse medical events,but receives insufficient support from the hospitals,so they are prone to job burnout,resigning or transferring to other jobs.Therefore,hospital administrations should pay more attention to the secondary victims by providing a strong support system and alleviating the painful experience of the second victim to reduce the resignation rate and ensure medical safety and satisfaction.
