摘 要:目的探讨直接屏气方式在降低冠状动脉CT血管成像(CCTA)辐射剂量的可行性.方法选取40例(心率<75次/min)临床拟诊冠心病行CCTA检查者.冠状动脉钙化积分(CS)采用吸气后屏气方式扫描,标记为A组;CCTA采用平静呼吸下直接屏气方式扫描,标记为B组.对比研究2组受检者平静呼吸时心率(基础心率)、扫描期间屏气时的心率,容积扫描长度及辐射剂量等.结果扫描期间屏气时心率明显低于基础心率(P<0.05),A组平均下降约(3.56±0.4)次/min,B组平均下降约(6.17±0.5)次/min.A组容积扫描长度128~160 mm,平均(142±10)mm,B组容积扫描长度100~140 mm,平均(120±11)mm,B组平均容积扫描长度比A组减少约15%,有效辐射剂量降低约31%(A组辐射剂量值以B组相同曝光参数输入CT机后模拟获得),差异有统计学意义(P<0.05).结论平静呼吸下直接屏气方式和吸气后屏气方式扫描时心率都有所下降,但直接屏气方式可减少容积扫描长度,从而有效降低辐射剂量,使受检查者受益.Objective To investigate the feasibility of direct breath-holding in reducing radiation dose of coronary CT angiography(CCTA).Methods Forty patients(heart rate<75 beats per minute)with suspected coronary heart disease were selected for CCTA examination.In group A:calcification score(CS)was scanned by breath-holding after inhalation;in group B:CCTA was scanned by direct breath-holding under calm breathing.The heart rate during calm breathing(basal heart rate),the heart rate during breath-holding,volume scanning length and radiation dose were compared between the two groups.Results The heart rate during breath-holding was significantly lower than that of the basal heart rate(P<0.05)during the scanning period.And the average decrease in group A was about(3.56±0.4)beats per minute,group B was(6.17±0.5)beats per minute.The volume scanning length of group A was 128-160 mm,mean(142±10)mm,group B was 100-140 mm,mean(120±11)mm.The average volume scanning length of group B was about 15%less than that of group A,effective radiation dose was reduced by about 31%(while the radiation dose of group A was obtained by simulating the same CT scanning protocol of group B),the difference between the two groups was statistically significant(P<0.05).Conclusion The heart rate decreased in both breath-holding after inhalation mode and direct breath-holding under calm breathing mode.However,the direct breath-holding mode can reduce the volume scanning length and effectively reduce the radiation dose,which benefits the examinees.
