摘 要:目的探讨血清胱抑素C(Cys-C)、补体C1q、转铁蛋白(TRF)、25羟基维生素D[25(OH)D]与糖尿病肾病(DN)的相关性。方法回顾性分析2019年1月至2020年12月期间北京中医药大学附属护国寺中医医院收治的168例2型糖尿病患者的临床资料,根据尿蛋白排泄率(UAER)分为糖尿病组(DM组)88例和糖尿病肾病组(DN组)80例。选取同期在我院体检的健康体检者100例作为对照组。比较三组受检者的Cys-C、C1q、TRF、25(OH)D水平,利用Spearman相关性分析其与糖尿病肾病的相关性。结果DN组患者的Cys-C、C1q、TNF水平分别为(3.77±0.47)mg/L、(308.03±30.35)ng/L、(1.78±0.56)mg/L,明显高于DM组的(1.89±0.36)mg/L、(246.49±22.56)ng/L、(10.13±2.32)mg/L和对照组的(0.54±0.15)mg/L、(178.34±16.53)ng/L、(1.13±0.32)mg/L,差异均有统计学意义(P<0.05);DN组患者的25(OH)D水平为(20.49±3.41)μg/L,明显低于DM组的(27.48±2.87)μg/L和对照组的(35.72±3.84)μg/L,差异均有统计学意义(P<0.05);经Spearman相关性分析结果显示,Cys-C、C1q、TRF、25(OH)D与糖尿病肾病密切相关(P<0.05)。结论血清Cys-C、C1q、TRF、25(OH)D水平与糖尿病肾病密切相关,Cys-C、C1q、TRF是疾病进程中的危险因素,25(OH)D则是保护因素。Objective To investigate the correlation between serum cystatin C(Cys-C),complement C1q,transferrin(TRF),25 hydroxyvitamin D[25(OH)D]and diabetic nephropathy(DN).Methods The clinical data of 168 patients with type 2 diabetes mellitus admitted to Huguosi Hospital of Traditional Chinese Medicine Affiliated to Beijing University of Traditional Chinese Medicine from January 2019 to December 2020 were retrospectively analyzed.According to urinary protein excretion rate(UAER),they were divided into diabetes mellitus group(DM group,88 cases)and diabetic nephropathy group(DN group,80 cases).During the same period,100 individuals received healthy physical examination in the hospital were selected as the control group.The levels of Cys-C,C1q,TRF,and 25(OH)D in the three groups were compared.The correlation between Cys-C,C1q,TRF,and 25(OH)D was analyzed by Spearman correlation.Results The levels of Cys-C,C1q,and TNF in DN group were(3.77±0.47)mg/L,(308.03±30.35)ng/L,and(1.78±0.56)mg/L,respectively,significantly higher than(1.89±0.36)mg/L,(246.49±22.56)ng/L,(10.13±2.32)mg/L in DM group and(0.54±0.15)mg/L,(178.34±16.53)ng/L,(1.13±0.32)in control group(P<0.05).The level of 25(OH)D in DN group was(20.49±3.41)μg/L,which was significantly lower than(27.48±2.87)μg/L in DM group and(35.72±3.84)μg/L in the control group(P<0.05).Spearman correlation analysis showed that Cys-C,C1q,TRF,25(OH)D were closely correlated with diabetic nephropathy(P<0.05).Conclusion The levels of serum Cys-C,C1q,TRF,and 25(OH)D are closely related to diabetic nephropathy.Cys-C,C1q,and TRF are risk factors,and 25(OH)D is a protective factor.
