摘 要:目的对比分析不同胰岛素泵基础量分段法对2型糖尿病患者血糖控制效果的优劣。方法采用前瞻性研究,将2017年9月—2021年4月由该院收治的2型糖尿病患者165例用随机数表法随机分为3组,分别为A组55例、B组55例和C组55例,所有患者均给予胰岛素泵治疗,分别给予3段、5段和24段3种不同胰岛素泵基础量分段法,比较3组患者的血糖控制效果。结果A组、B组、C组3组患者治疗前的血糖值比较差异无统计学意义(P>0.05);治疗后相比治疗前的清晨空腹、三餐后2 h和凌晨3点血糖值均显著降低,差异有统计学意义(P<0.05),但3组间治疗后的血糖值比较差异无统计学意义(P>0.05)。平均胰岛素日使用剂量[(44.28±5.81)U/d vs(41.84±4.52)U/d vs(36.15±4.10)U/d]和血糖值控制至标准范围所需时间[(5.3±1.5)d vs(3.6±1.1)d vs(2.7±1.8)d]方面3组间比较差异有统计学意义(F=40.449、42.933,P<0.001),其中B组和C组血糖值达标时间和胰岛素用量均明显短于和少于A组,C组血糖值达标时间和胰岛素用量也明显短于和少于B组,差异有统计学意义(P<0.05)。低血糖发生情况的比较中,C组明显低于A组(1.8%vs 16.4%),差异有统计学意义(χ^(2)=5.390,P=0.020)。结论3段、5段和24段3种不同胰岛素泵基础量分段法对2型糖尿病患者血糖控制效果相当,但在胰岛素应用剂量、血糖控制至标注那范围所需时间及低血糖发生这3个方面有明显不同,其中24段法相比较而言的效果最优,但其设置方法繁琐,在具体选择何种基础量分段法时需要综合考虑患者的一般情况和病情决定。Objective To compare and analyze the advantages and disadvantages of different insulin pump basal volume segmentation methods on blood glucose control in patients with type 2 diabetes mellitus.Methods Using a prospective study,165 patients with type 2 diabetes mellitus who were admitted to the hospital from September 2017 to April 2021 were randomly divided into 3 groups by random number table method,with 55 cases in group A,55 cases in group B and 55 cases in group C respectively.All patients were treated with insulin pump,and 3 different insulin pump basal volume segmentation methods were given respectively,and the blood glucose control effect of the three groups was compared.Results There was no statistically significant difference in the blood glucose levels between groups A,B,and C before treatment(P>0.05).After treatment,compared with before treatment,fasting in the morning,2 h after three meals and 3 o'clock in the morning blood glucose values were significantly lower,the difference was statistically significant(P<0.05),but there was no significant difference in blood glucose levels between the three groups after treatment(P>0.05).The average daily insulin dose[(44.28±5.81)U/d vs(41.84±4.52)U/d vs(36.15±4.10)U/d]and the time required to control the blood glucose value to the standard range[(5.3±1.5)d vs(3.6±1.1)d vs(2.7±1.8)d],there were statistically significant differences among the three groups(F=40.449,42.933,P<0.001),among which the time to reach the blood glucose level and the insulin dosage in groups B and C Both were significantly shorter and less than those of group A,and the time to reach the blood glucose level and the dosage of insulin in group C were also significantly shorter and less than those in group B,and the difference was statistically significant(P<0.05).In the comparison of hypoglycemia,group C was significantly lower than group A(1.8%vs16.4%),and the difference was statistically significant(χ^(2)=5.390,P=0.020).Conclusion The 3-stage,5-stage and 24-stage basal volume fraction
