摘　　要：目的探讨分析益气养阴清热活血汤对早期糖尿病肾病的临床应用价值及对超敏C反应蛋白、尿白蛋白排泄率的影响。方法选取该院2018年10月—2020年10月收治的早期糖尿病肾病患者90例,采用随机数表法均分为两组,各45例。中药组在常规治疗基础上采用益气养阴清热活血汤治疗,常规组仅进行常规治疗;评价患者的临床疗效,检测患者治疗前后的血糖水平、血清炎性因子及超敏C反应蛋白水平、肾功能相关指标水平,调查患者治疗前后的生活质量评分,观察患者的不良反应,对两组进行比较。结果中药组的临床总有效率为95.56%,常规组的临床总有效率为77.78%,中药组明显高于常规组,差异有统计学意义(χ^(2)=6.179,P<0.05)。治疗前,两组的FPG、2 hPG、HbA1c水平比较,差异无统计学意义(t=0.212、0.085、0.237,P>0.05),治疗后,两组再比较示中药组均明显低于常规组,差异有统计学意义(t=10.652、5.679、12.204,P<0.05);治疗前,两组的IL-18、TNF-α、hs-CRP水平比较差异无统计学意义(t=0.156、0.206、0.089,P>0.05);治疗后,两组比较,中药组均明显低于常规组,差异有统计学意义(t=25.952、18.258、11.713,P<0.05)。治疗前,两组的Scr、BUN、UAER水平比较,差异无统计学意义(t=0.356、0.025、0.088,P>0.05);治疗后,两组比较,中药组均明显低于常规组,差异有统计学意义(t=30.692、9.327、23.350,P<0.05)。治疗前,两组的生活质量各维度评分比较,差异无统计学意义(t=0.143、0.153、0.316、0.160,P>0.05);治疗后,两组比较,中药组均明显高于常规组,差异有统计学意义(t=28.125、20.202、17.482、27.219,P<0.05)。两组在治疗期间均未发生严重不良反应。结论采用益气养阴清热活血汤治疗早期糖尿病肾病可取得较好的临床疗效,使患者的超敏C反应蛋白、尿白蛋白排泄率等指标水平得到有效改善,值得推广。Objective To investigate the clinical application value of Yiqi Yangyin Qingre Huoxue Decoction on early diabetic nephropathy and its effect on high-sensitivity C-reactive protein and urinary albumin excretion rate.Methods A total of 90 patients with early diabetic nephropathy who were admitted to the hospital from October 2018 to October 2020 were selected and divided into two groups by random number table method,with 45 cases in each group.Traditional Chinese medicine group group was treated with Yiqi Yangyin Qingre Huoxue Decoction on the basis of conventional treatment,and conventional group only received conventional treatment.The clinical efficacy of the patients was evaluated,and the blood glucose levels,serum inflammatory factors and high-sensitivity C-reactive protein levels,and renal function-related index levels were detected before and after treatment.The quality of life scores of the patients before and after treatment were investigated,and the adverse reactions of the patients were observed,and the two groups were compared.Results The total clinical effective rate of the traditional Chinese medicine group was 95.56%,and that of the routine group was 77.78%,the traditional Chinese medicine group was significantly higher than the routine group,and the difference was statistically significant(χ^(2)=6.179,P<0.05).There was no statistically significant difference in the levels of FPG,2 hPG and HbA1c between the two groups before treatment(t=0.212,0.085,0.237,P>0.05).After treatment,the comparison between the two groups showed that the traditional Chinese medicine group was significantly lower than the conventional group,and the difference was statistically significant(t=10.652,5.679,12.204,P<0.05).There was no statistically significant difference in the levels of IL-18,TNF-αand hs-CRP between the two groups before treatment(t=0.156,0.206,0.089,P>0.05).After treatment,the comparison between the two groups showed that the traditional Chinese medicine group was significantly lower than the routine group,a
