摘 要:目的探讨基于新媒体的健康促进模式在社区慢病管理中的应用。方法选取2019年5月—2020年5月新店卫生院所在社区的200例糖尿病患者为研究对象,将患者随机分为研究组和参照组,每组100例,参照组采用常规慢病管理模式,研究组采用基于新媒体的健康促进模式,观察不同管理模式的效果。结果干预前,两组生活能力评分比较,差异无统计学意义(P>0.05);干预后,研究组生活能力显著高于参照组,差异有统计学意义(P<0.05)。干预前,两组健康依从性比较,差异无统计学意义(P>0.05);干预后,研究组健康依从性显著高于参照组,差异有统计学意义(P<0.05)。干预前,参照组空腹血糖为(14.7±2.3)mmol/L,研究组空腹血糖为(15.5±1.6)mmol/L,两组比较差异无统计学意义(P>0.05);干预后,参照组空腹血糖为(9.6±1.5)mmol/L,研究组空腹血糖为(6.7±1.4)mmol/L,两组血糖水平均比干预前显著降低,且研究组显著低于参照组,差异有统计学意义(P<0.05)。结论基于新媒体的健康促进模式能够改善社区慢病患者的健康依从性及生活能力,适于临床推广。Objective To explore the application of new media-based health promotion model in community chronic disease management.Methods A total of 200 diabetic patients in the community where Xindian Health Center was located from May 2019 to May 2020 were selected as the research objects,and the patients were randomly divided into a research group and a reference group,with 100 cases in each group.The reference group adopted the conventional chronic disease management model,and the group adopted a new media-based health promotion model to observe the effects of different management models.Results Before the intervention,there was no statistically significant difference in the scores of living ability between the two groups(P>0.05).After the intervention,the living ability of the study group was significantly higher than that of the reference group,and the difference was statistically significant(P<0.05).Before the intervention,there was no statistically significant difference in the health compliance between the two groups(P>0.05);after the intervention,the health compliance of the study group was significantly higher than that of the reference group,and the difference was statistically significant(P<0.05).Before the intervention,the fasting blood glucose of the reference group was(14.7±2.3)mmol/L,and the fasting blood glucose of the study group was(15.5±1.6)mmol/L,and the difference was not statistically significant between two groups(P>0.05);after the intervention,the fasting blood glucose in the reference group was(9.6±1.5)mmol/L,and the fasting blood glucose in the study group was(6.7±1.4)mmol/L,the blood glucose levels in the two groups were significantly lower than those before the intervention,and the study group was significantly lower than the reference group,the difference was statistically significant(P<0.05).Conclusion The health promotion model based on new media can improve the health compliance and life ability of patients with chronic diseases in the community,and is suitable for clinical promotion.
