摘　　要：目的探究不同类型减重代谢手术治疗重度肥胖合并2型糖尿病对体质量、体质指数(BMI)、空腹血糖及空腹胰岛素的价值。方法选取2020年1月—2021年6月该院接收的80例重度肥胖合并2型糖尿病患者进行研究,根据手术方式的不同来进行分组,将其分为胃袖状切除组和胃转流组,各40例。对两组患者进行观察,分析其体质量、BMI、空腹血糖及空腹胰岛素的变化情况。结果两组患者的手术均顺利完成。两组患者在术后6个月、12个月的体质量和BMI值均低于术前,差异有统计学意义(P<0.05);两组患者在术后6个月、12个月的空腹血糖值和空腹胰岛素值均低于术前,差异有统计学意义(P<0.05)。结论腹腔镜胃袖状切除手术和腹腔镜胃转流手术对重度肥胖合并2型糖尿病的治疗效果都较理想,能够改善患者的临床症状,提升其生活质量。Objective To explore the value of different types of bariatric and metabolic surgery in the treatment of severe obesity complicated with type 2 diabetes on body weight,body mass index(BMI),fasting blood glucose and fasting insulin.Methods 80 cases of severe obesity combined with type 2 diabetes from January 2020 to June 2021 were selected.According to different surgical methods,they were divided into gastric sleeve resection group and gastric bypass group,with 40 cases in each group.The two groups of patients were observed,and the changes of body weight,BMI,fasting blood glucose and fasting insulin were analyzed.Results The operations of the two groups were successfully completed.The body weight and BMI of the two groups at 6 months and 12 months after surgery were lower than those before surgery,and the difference was statistically significant(P<0.05);the fasting blood glucose and fasting insulin values in the two groups at 6 months and 12 months after operation were lower than those before operation,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic gastric sleeve resection and laparoscopic gastric bypass surgery are ideal for the treatment of severe obesity complicated with type 2 diabetes.They can improve the clinical symptoms of patients and improve their quality of life.
