摘 要:目的分析糖尿病周围神经病变应用剪切波超声弹性成像检查的诊断价值。方法选取该院2020年3月—2021年5月收治的2型糖尿病患者85例为研究对象,将其中不伴周围神经病变的40例患者设为非DPN组,将另外伴周围神经病变的45例患者设为DPN组;另选取同期在该院接受健康体检的40名健康体检人员,将其设为对照组。全部研究对象均接受剪切波弹性成像检查,对胫神经杨氏模量平均值(Emean)、杨氏模量平最大值(Emax)、横截面积(CSA)进行测量,观察比较测量结果。结果DPN组的Emean、Emax及CSA均明显大于非DPN组、对照组,差异有统计学意义(P<0.05);非DPN组的Emean、Emax及CSA均明显大于对照组,差异有统计学意义(P<0.05)。CSA诊断价值明显低于Emean、Emax,差异有统计学意义(P<0.05)。结论采用剪切波超声弹性成像检查能对神经硬度改变情况进行量化,进而对糖尿病周围神经病变进行准确评估,在诊断糖尿病周围神经病变时具有非常重要的价值。Objective To analyze the diagnostic value of shear wave ultrasound elastography in diabetic peripheral neuropathy.Methods A total of 85 patients with type 2 diabetes who were admitted to the hospital from March 2020 to May 2021 were selected.The 40 patients without peripheral neuropathy were set as the non-DPN group,and the other 45 patients with peripheral neuropathy were set as the DPN group.In addition,40 healthy people who received health checkup in the hospital during the same period were selected and set as the control group.All subjects received shear wave elastography.The average young's modulus(Emean),maximum(Emax)Young's modulus and cross-sectional area(CSA)of tibial nerve were measured,and the measured results were observed and compared.Results Emean,Emax and CSA of DPN group were significantly higher than those of non-DPN group and control group,and the difference was statistically significant(P<0.05).The Emean,Emax and CSA of the non-DPN group were significantly greater than those of the control group,and the difference was statistically significant(P<0.05).The diagnostic value of CSA was significantly lower than that of Emean and Emax,and the difference was statistically significant(P<0.05).Conclusion Using shear wave ultrasound elastography can quantify changes in nerve stiffness,and then accurately assess diabetic peripheral neuropathy.It is of great value in the diagnosis of diabetic peripheral neuropathy.
