柴芩温胆汤加减治疗糖尿病胃轻瘫的临床疗效观察

摘　　要：目的探讨柴芩温胆汤加减治疗糖尿病胃轻瘫的临床疗效,分析其对患者血糖水平和胃肠功能障碍的影响。方法选择该院2019年1月—2021年6月收治的60例糖尿病胃轻瘫患者作为该次研究对象,以计算机表法的形式将患者分为研究组和对照组,对照组实施多潘立酮片治疗,研究组在此基础上加用柴芩温胆汤加减治疗,比较两组患者中医证候积分评分与总积分、血糖指标、胃电图、临床有效率、不良反应发生情况、胃肠功能指标(MOT、GAS、CCK)、胃排空时间、匹茨堡睡眠质量指数量表(PSQI)评分与血液流变学指标(WBV、FOB)。结果研究组治疗总有效率(93.33%)高于对照组(70.00%),研究组胃窦部胃电频率、胃窦部胃电振幅、胃电节律高于对照组,研究组不良反应发生率(10.00%)低于对照组(33.33%),研究组胃排空时间、PSQI评分、WBV、FOB均优于对照组,研究组血清MOT、GAS水平高于对照组,CCK水平低于对照组,研究组空腹血糖、餐后2 h血糖、糖化血红蛋白低于对照组,研究组中医证候积分与总积分显著低于对照组,差异有统计学意义(P<0.05)。结论柴芩温胆汤加减治疗糖尿病胃轻瘫的临床价值显著,可以改善患者血糖水平,改善胃肠功能,值得应用。Objective To explore the clinical efficacy of Chaiqin Wendan Decoction in the treatment of diabetic gastroparesis,and to analyze the effects of patients'blood glucose levels and gastrointestinal dysfunction.Methods Sixty patients with diabetic gastroparesis admitted to the hospital from January 2019 to June 2021 were selected as the study object.The patients were divided into study group and control group in the form of computer table.The control group was given domperidone tablets,and the study group was treated with Chaiqin Wendan Decoction on this basis.The scores of TCM syndrome scores and total scores,blood glucose indexes,electrogastrogram,clinical effective rate,occurrence of adverse reactions,gastrointestinal function indexes(MOT、GAS、CCK),gastric emptying time,pittsburgh sleep quality index(PSQI)score and hemorheology indexes(WBV、FOB)were compared between the two groups of patients.Results The total effective rate of treatment in the study group(93.33%)was higher than that in the control group(70.00%);the frequency,amplitude and rhythm of gastric electricity in the antrum of the study group were higher than those in the control group;the incidence of adverse reactions in the study group(10.00%)was lower than that in the control group(33.33%);the gastric emptying time,PSQI score,WBV,and FOB of the study group were all better than those in the control group;the serum MOT and GAS levels of the study group were higher than those of the control group,and the CCK level was lower than that of the control group;the fasting blood glucose,2 h postprandial blood glucose,and glycosylated hemoglobin of the study group were lower than those of the control group;the TCM syndrome scores and total scores of the study group were significantly lower than those of the control group,the difference was statistically significant(P<0.05).Conclusion Chaiqin Wendan Decoction has significant clinical value in the treatment of diabetic gastroparesis.It can improve the blood glucose level and gastrointestinal function of patien
