摘　　要：目的:探讨环氧合酶-2(COX-2)和基质金属蛋白酶-14(MMP-14)在子宫内膜异位症(EMs)组织中表达及与血清糖类抗原125(CA125)相关性。方法:选择2017年3月-2020年3月在本院妇产科行腹腔镜手术的EMs患者115例作为EMs组,同期因子宫肌瘤行手术治疗并经病理证实子宫内膜无异常的50例作为对照组,采用实时荧光定量PCR检测两组子宫内膜组织中COX-2、MMP-14 mRNA水平,免疫组化法检测COX-2和MMP-14蛋白表达水平,Pearson法分析COX-2、MMP-14表达水平与患者血清CA125相关性,分析COX-2、MMP-14表达对EMs诊断价值。结果:EMs组在位内膜组织COX-2(1.45±0.26)、MMP-14 mRNA(1.53±0.25)及血清CA125(36.47±9.87 IU/ml)水平均升高,COX-2、MMP-14蛋白阳性表达率均高于对照组(均P<0.05);EMs组COX-2、MMP-14表达与血清CA125水平呈正相关(r=0.619、0.515,均P<0.05);COX-2、MMP-14联合预测Ems的曲线下面积为0.913,敏感度86.1%,特异度为86.0%。结论:COX-2、MMP-14在Ems在位内膜组织中高表达,且与患者血清CA125水平正相关,联合检测对EMs早期诊断有一定价值。Objective:To detect the expressions of cyclooxygenase-2(COX-2)and matrix metalloproteinase-14(MMP-14)in endometriosis tittuse of patients,and to explore the correlation between the levels of COX-2 and MMP-14 of patients and their serum carbohydrate antigen 125(CA125)level.Methods:115 patients with endometriosis who wanted laparoscopic surgery were selected in observation group from March 2017 to March 2020,and 50 patients who had accepted hysteromyoma and were confirmed no abnormality of endometrium by pathology were selected in control group during the same time.The mRNA levels of COX-2 and MMP-14 in endometrial tissue of the patients in the two groups were detected by real-time fluorescent quantitative PCR(qRT-PCR),and their protein expressions of COX-2 and MMP-14 were detected by immunohistochemistry.Pearson method was used to analyze the correlation between the expressions of COX-2 and MMP-14 of the patients and their CA125 level.The value of COX-2 and MMP-14 expressions for diagnosing endometriosis was analyzed.Results:The mRNA levels of COX-2(1.45±0.26)and MMP-14(1.53±0.25),and the serum CA125 level(36.47±9.87 IU/mL)in the endometrial tissue of the patients in the observation group had increased significantly.The positive rates of protein levels of COX-2 and MMP-14 of the patients in the observation group was significantly higher than those of the patients in the control group(all P<0.05).In observation group,the expressions of COX-2 and MMP-14 of the patients were positively correlated with their serum CA125 level(r=0.619,0.515,all P<0.05).The area under the curve,the sensitivity,and the specificity of the combined Cox-2 and MMP-14 levels for predicting endometriosis were 0.913,86.1%,and 86.0%,respectively.Conclusion:COX-2 and MMP-14 are highly expressed in the endometrial tissues of the patients with endometriosis,and which are positively correlated with the serum CA125 level of the patients.Combined detection of COX-2 and MMP-14 has certain value for early diagnosing endometriosis,and which is expecte
