骨科手术中无菌手套微小穿孔与细菌污染情况分析

摘　　要：目的 探讨骨科手术使用的无菌手套在不同时间点微小穿孔与细菌污染情况。方法 选取在某医院手术室骨科手术使用的280个无菌手套作为研究对象,重点对无菌手套不同时间点微小穿孔与细菌污染情况进行统计分析。结果骨科手术使用的280个无菌手套中,手术120分钟时开始有细菌检出,手术180分钟时有59个(21.07%)无菌手套检出细菌,共培养出75株病原菌,最常见的三种病原菌为凝固酶阴性葡萄球菌21株(28.0%)、金黄色葡萄球菌15株(20.0%)与大肠埃希菌14株(18.67%)。手术开始180分钟时,骨科手术使用的280个无菌手套中,共有54个(19.29%)发生了微小穿孔,不同手术角色使用的无菌手套微小穿孔发生率存在着显著差异(p<0.05);骨科手术发生微小穿孔的无菌手套细菌检出率显著高于未发生微小穿孔的手套(p<0.01)。结论 骨科手术最好在120分钟内更换无菌手套,微小穿孔和主刀医生的无菌手套细菌污染率更高,凝固酶阴性葡萄球菌是术中污染无菌手套最常见的病原菌,值得关注。Objective To investigate bacterial contamination and micro-perforation of sterile gloves used in orthopedic surgery at different time points.Methods A total of 280 sterile gloves used in orthopedic surgery in the operating room of a hospital were selected as the research objects,and the bacterial contamination and micro-perforation of sterile gloves at different time points were analyzed.Results Among the 280 sterile gloves used in orthopedic surgery,bacteria were initially detected at 120mins after surgery,and were found in 59(21.07%) gloves at 180mins 75 strains of pathogenic bacteria cultured.The three most common pathogenic bacteria were as follows:coagulase negative staphylococcus 21(28.0%),staphylococcus aureus 15(20.0%) and escherichia coli 14(18.67%).At 180mins after surgery,54(19.29%) of the 280 sterile gloves had microperforation,and there was a significant difference in the incidence of micro-perforation among different users(p<0.05).The bacteria detection rate of gloves with micro-perforation was significantly higher than that of gloves without micro-perforation in orthopedic surgery(p<0.01).Conclusion In orthopedic surgery,it is recommended to replace sterile gloves within 120mins.Bacterial contamination rate in the sterile gloves of micro-perforation and of the surgeons is higher.Coagulase negative staphylococcus is the most common pathogen on contaminated sterile gloves in surgery,which deserves attention.
