摘  要:目的:研究β_(2)微球蛋白(β_(2)-microglobulin,β_(2)-MG)、微小RNA-182(miR-182)、血尿素氮(Blood ureanitrogen,BUN)在老年慢性肾功能衰竭中的表达及相关性。方法:选择2019年3月至2020年12月接受治疗的慢性肾功能衰竭患者43例,作为研究组;选取于同时期进行健康体检的正常人43例,作为对照组。血清miR-182表达采用荧光定量聚合酶链式反应技术检测,采用免疫比浊法检测β_(2)-MG、BUN水平,采用酶联免疫吸附测定法检测血清CYSC、Scr、RBP水平,受试者特征曲线(receiver operator characteristics curve,ROC)分析β_(2)-MG、miR-182、BUN在老年慢性肾功能衰竭中诊断价值。结果:研究组β_(2)-MG、miR-182、BUN、CYSC、Scr、RBP表达水平均高于对照组(P<0.05)。相关性分析显示,β_(2)-MG与miR-182之间呈正相关(r=0.695,P=0.001);β_(2)-MG与BUN之间呈正相关(r=0.733,P=0.001);miR-182与BUN之间呈现正相关(r=0.778,P=0.001)(P<0.05)。研究组早期患者β_(2)-MG、miR-182、BUN水平高于非早期患者(P<0.05)。年龄≥65岁、少尿、水肿、大量尿蛋白老年慢性肾功能衰竭患者β_(2)-MG、miR-182、BUN表达水平较高(P<0.05)。ROC分析β_(2)-MG、miR-182、BUN诊断老年慢性肾功能衰竭灵敏度低于三项联合(P<0.05)。结论:β_(2)-MG、miR-182、BUN在老年慢性肾功能衰竭中高表达,且三者之间具有相关性,在老年慢性肾功能衰竭的诊断中具有一定的临床价值。Objective:To study the expression and correlation ofβ_(2) microglobulin(β_(2)-microglobulin,β_(2)-MG),microRNA-182(miR-182),and blood ureanitrogen(BUN)in elderly chronic renal failure.Methods:Totally 43 patients with chronic renal failure treated from March 2019 to December 2020 were selected as the study group;43 normal subjects who underwent physical examination in the same period were selected as the control group.Serum miR-182 expression was detected by fluorescence quantitative polymerase chain reaction technique,β_(2)-MG and BUN levels were detected by immunoturbidimetric method,serum CYSC,Scr and RBP levels were detected by enzyme-linked immunosorbent assay,and subject operator characteristics curve(ROC)was used to analyze the diagnostic value ofβ_(2)-MG,miR-182 and BUN in chronic renal failure in the elderly.Results:The expression levels ofβ_(2)-MG,miR-182,and BUN in the study group were higher than those in the control group(P<0.05).Correlation analysis showed a positive correlation betweenβ_(2)-MG and miR-182(r=0.695,P=0.001);betweenβ_(2)-MG and BUN(r=0.733,P=0.001);and between miR-182 and BUN(r=0.778,P=0.001)(P<0.05).The levels ofβ_(2)-MG,miR-182 and BUN were higher in early stage patients than in non-early stage patients in the study group(P<0.05).The expression levels ofβ_(2)-MG,miR-182 and BUN were higher in elderly patients with chronic renal failure aged≥65 years,with oliguria,oedema and large amounts of urine protein(P<0.05).ROC analysis ofβ_(2)-MG,miR-182 and BUN was less sensitive than the three combined in diagnosing elderly chronic renal failure(P<0.05).Conclusion:β_(2)-MG,miR-182,and BUN are highly expressed in elderly chronic renal failure,and there is a correlation between the three,which has certain clinical value in the diagnosis of elderly chronic renal failure.
