摘　　要：目的:以科层制理论为基础,了解北京市公立医院科层管理现状对医患信任的影响因素,探讨构建和谐医患信任的途径与对策。方法:选取北京优质医疗资源集中、患者就医集中、医院优势特色专科及科层组织建立时间可比较的6家三级公立医院,通过实地观察、个人访谈等方法进行研究。结果:公立医院组织架构彰显科层层级特征,其医疗服务制度化和规范化,建树了各自品牌和声誉,赢得了患者的信任。医患信任以普遍主义信任为基本特征,但在生物医学基础上建立的科层制,其纷繁专业的建立和去人格化的特点,也让患者产生信任质疑。结论:科层组织架构与层级管理是目前公立医院的基本运行方式,研究科层制视角下医患信任关系,有助于以患者为中心,整体提升公立医院医疗服务水平。Objective:Based on the theory of bureaucratic system,to understand the influencing factors of the current bureaucratic management of public hospitals in Beijing on the doctor-patient trust,and to explore the ways and countermeasures to build harmonious doctor-patient trust.Methods:Select 6 Grade III public hospitals in Beijing with concentrated high-quality medical resources,concentrated patients seeking medical treatment,and comparable specialties and bureaucratic organizations in Beijing,and conduct research through field observation and personal interviews.Results:The organizational structure of public hospitals showed the characteristics of bureaucratic level,and their medical services were institutionalized and standardized,built their own brands and reputations,and won the trust of patients.The basic feature of doctor-patient trust is universal trust,but the bureaucratic system established on the basis of biomedicine,the establishment of various specialties and the characteristics of depersonalization,also make patients question trust.Conclusion:The bureaucratic organizational structure and hierarchical management are the basic operation methods of public hospitals at present.To study the trust relationship between doctors and patients from the perspective of bureaucratic system is helpful to take patients as the center and improve the medical service level of public hospitals as a whole.
