摘 要:目的探讨血细胞分析仪检测未成熟血小板分数(IPF)在儿童血小板减少性疾病中的应用价值。方法选取2020年10月至2021年10月该院收治的免疫性血小板减少症(ITP)患儿20例(ITP组)、再生障碍性贫血(AA)患儿20例(AA组)、急性白血病(AL)患儿20例(AL组)、健康儿童20例(健康对照组)作为研究对象,采用血细胞分析仪检测各组研究对象血小板相关参数,如血小板计数(Plt)、IPF等。比较各组研究对象IPF的差异。采用受试者工作特征曲线分析IPF在ITP与各组疾病鉴别中的价值。结果ITP组、AA组、AL组患儿Plt均明显低于健康对照组,IPF均明显高于健康对照组,差异均有统计学意义(P<0.05)。ITP组患儿IPF均明显高于健康对照组、AA组、AL组,差异均有统计学意义(P<0.05)。当IPF临界值取5.80%时鉴别诊断ITP与AA曲线下面积为0.995,灵敏度为100%,特异度为95%。当IPF临界值取5.70%时鉴别诊断ITP与AA曲线下面积为0.995,灵敏度为100%,特异度为95%。结论血液分析仪检测IPF在鉴别儿童血小板减少性疾病中具有一定的应用价值。Objective To explore the application value of the blood cell analyzer in detecting immature platelet fraction(IPF)in the children with thrombocytopenic disorders.Methods From October 2020 to October 2021,20 children with immune thrombocytopenia(ITP group),20 children with aplastic anemia(AA group),20 children with acute leukaemia(AL group)and 20 healthy children(healthy control group)admitted to the hospital were selected as the study subjects.Blood cell analyzer was used to detect platelet related parameters,such as platelet count(Plt),IPF,etc.The differences in IPF between the study groups were compared.Receiver operating characteristic curve was used to analyze the value of IPF in differentiating ITP from various diseases.Results The Plt of the children in the ITP group,AA group and AL group was significantly lower than that of the healthy control group,and the IPF of the above groups was significantly higher than that of the healthy control group.The differences were statistically significant(P<0.05).The IPF of the ITP group was significantly higher than that of the healthy control group,AA group and AL group.The differences were statistically significant(P<0.05).When the IPF boundary value was 5.80%,the area under the differential diagnosis ITP and AA curve was 0.995,the sensitivity was 100%,and the specificity was 95%.When the IPF boundary value was 5.70%,the area under the differential diagnosis ITP and AA curve was 0.995,the sensitivity was 100%,and the specificity was 95%.Conclusion The haematology analyzer test for IPF is of value in identifying the thrombocytopenic disorders in children.
