摘 要:目的探讨血清可溶性血管细胞粘附分子1(sVCAM-1)、微小RNA-126水平(miR-126)与维持性血液透析患者动静脉内瘘狭窄的相关性。方法选取2017年3月至2019年3月于本院行维持性血液透析的200例患者,根据动静脉内瘘有无发生狭窄将其分为内瘘狭窄组(62例)和内瘘非狭窄组(138例)。采用酶联免疫吸附法(ELISA)检测患者血清sVCAM-1水平,采用实时荧光定量PCR(qRT-PCR)检测患者血清miR-126水平,同时采用自动生化仪检测总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血红蛋白(Hb)、血肌酐(Scr)、血钙、血磷、尿素氮(BUN)、白蛋白(ALB)、C反应蛋白(CRP)等生化指标;分析维持性血液透析动静脉内瘘狭窄与血清sVCAM-1、miR-126水平的相关性及二者的诊断价值;分析影响维持性血液透析患者动静脉内瘘狭窄的因素。结果内瘘狭窄组的CRP、sVCAM-1水平均高于内瘘非狭窄组(均P<0.05),miR-126水平低于内瘘非狭窄组(P<0.05);维持性血液透析患者动静脉内瘘发生狭窄与血清sVCAM-1、miR-126水平呈负相关(r=-0.600,P<0.05);血清sVCAM-1、miR-126水平诊断动静脉内瘘狭窄的受试者工作特征(ROC)曲线下面积(AUC)分别为0.813、0.882,特异度分别为78.3%、75.4%,灵敏度分别为72.6%、87.1%;二者联合诊断的AUC为0.931,特异度为86.2%,灵敏度为87.1%;sVCAM-1高表达、miR-126低表达是维持性血液透析患者动静脉内瘘发生狭窄的危险因素。结论血清sVCAM-1、miR-126水平与维持性血液透析患者动静脉内瘘发生狭窄有密切联系。Objective To explore the correlations between the levels of serum soluble vascular cell adhesion molecule-1(sVCAM-1)and microRNA-126(miR-126)and arteriovenous fistula stenosis in maintenance hemodialysis.Methods From March 2017 to March 2019,200 patients who were hospitalized in our hospital for maintenance hemodialysis were divided into internal fistula stenosis group(62 cases)and internal fistula non stenosis group(138 cases).The general data of the two groups were analyzed;the level of serum sVCAM-1 was measured by enzyme-linked immunosorbent assay(ELISA),the serum miR-126 level was detected by real-time fluorescent quantitative PCR(qRT-PCR),at the same time,total cholesterol(TC),triglyceride(TG),high density lipoprotein choles-terol(HDL-C),low density lipoprotein cholesterol(LDL-C),hemoglobin(HB),serum creatinine(Scr),blood calcium,blood phosphorus,urea nitrogen(BUN),albumin(ALB),C-reactive protein(CRP)and other biochemical indexes were detected by automatic biochemical instrument;the correlation between the levels of serum sVCAM-1 and miR-126 in patients with arteriovenous fistula stenosis during maintenance hemodialysis and the diagnostic values was analyzed;and the factors that affect the stenosis of arteriovenous fistula in maintenance hemodialysis patients were analyzed.Results The levels of CRP and sVCAM-1 in the stenosis group were significantly higher than those in the non stenosis group(all P<0.05),and miR-126 was significantly lower than that in the non stenosis group(P<0.05);the levels of serum sVCAM-1 and miR-126 were negatively correlated in patients with arteriovenous fistula stenosis(r=-0.600,P<0.05);the area under curve(AUC)of levels of serum sVCAM-1 and miR-126 in the diagnosis of arteriovenous fistula stenosis in maintenance hemodialysis patients were 0.813 and 0.882 respectively,the specificities were 78.3%and 75.4%respectively,and the sensitivities were 72.6%and 87.1%respectively;the AUC of the combination in the diagnosis of arteriovenous fistula stenosis in maintenance hemodialysis patie
