摘　　要：目的探究阿昔洛韦联合免疫球蛋白治疗病毒性脑膜炎(VM)对患者T细胞亚群和炎症因子水平的影响。方法选取2018年2月至2019年10月于本院接受治疗的86例VM患者作为研究对象,采用随机信封法分为单一组与联合组,每组43例。单一组给予阿昔洛韦治疗,联合组在对照组基础上给予免疫球蛋白治疗,比较两组临床疗效及T细胞亚群、炎症因子水平。结果联合组治疗总有效率明显高于单一组,差异有统计学意义(P<0.05)。治疗后,联合组血清与脑脊液中CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均明显高于单一组,CD8^(+)水平低于单一组,差异有统计学意义(P<0.05)。联合组血清与脑脊液中降钙素原(PCT)、γ-干扰素(INF-γ)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均明显低于单一组,差异有统计学意义(P<0.05)。结论阿昔洛韦联合免疫球蛋白治疗病毒性脑膜炎效果显著,可有效减轻患者炎症反应,加强机体免疫水平,改善预后。Objective To explore the effects of acyclovir combined with immunoglobulin in the treatment of viral meningitis(VM)on T cell subsets and inflammatory factors levels in patients.Methods 86 patients with VM treated in our hospital from February 2018 to October 2019 were selected as the research subjects,and they were divided into single group and combined group according to random envelope method,with 43 cases in each group.The single group was treated with acyclovir,and the combined group was treated with immunoglobulin on the basis of the control group,the clinical efficacy,T cell subsets and inflammatory factors were compared between the two groups.Results The total effective rate of the combined group was significantly higher than that of the single group,and the difference was statistically significant(P<0.05).After treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in serum and cerebrospinal fluid in the combined group were significantly higher than those in the single group,and the levels of CD8+was lower than that in the single group,and the difference was statistically significant(P<0.05).The serum and cere-brospinal fluid levels of procalcitonin(PCT),interferon-γ(INF-γ),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the combined group were significantly lower than those in the single group,and the difference was statistically significant(P<0.05).Conclusion Acyclovir combined with immunoglobulin has significant effect in the treatment of viral meningitis,which can effectively reduce the inflammatory reaction,strengthen the immune level and improve the prognosis.
