摘 要:目的 探讨关节镜下微创疗法对胫骨平台骨折患者膝关节功能、疼痛应激的影响。方法 选取2018年3月至2020年4月本院收治的80例胫骨平台骨折患者作为研究对象,按照随机数字表法分为对照组和观察组,每组40例。对照组采用切开复位内固定术治疗,观察组采用关节镜下微创疗法治疗。比较两组术前及术后3个月Lysholm膝关节评分量表评分、术前及术后7 d疼痛应激指标[P物质(SP)、神经肽Y(NPY)]、术后3个月内并发症发生情况。结果 术后3个月,两组Lysholm评分均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。术后7 d,两组SP、NPY水平均高于术前,但观察组低于对照组,差异有统计学意义(P<0.05)。术后3个月内,观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论 关节镜下微创疗法可改善胫骨平台骨折患者膝关节功能,减轻疼痛,减少术后并发症发生。Objective To investigate the effect of arthroscopic minimally invasive therapy on knee joint function and pain stress in patients with tibial plateau fractures. Methods A total of 80 patients with tibial plateau fractures in our hospital from March 2018 to April 2020 were selected as the research subjects, and they were divided into control group and observation group according to random number table method, with 40 cases in each group. The control group was treated with open reduction and internal fixation, and the observation group was treated with arthroscopic minimally invasive therapy. The Lysholm knee joint score scale before operation and 3 months after operation, the pain stress indexes(substance P [SP],neuropeptide Y [NPY]) before and 7 d after operation, and complications occurred within 3 months after operation were compared between the two groups. Results At 3 months after operation, the Lysholm scores of the two groups were higher than those before the operation, and the observation group was higher than those in the control group, and the difference was statistically significant(P<0.05). At 7 d after operation, the levels of SP and NPY in the two groups were higher than those before operation, but the observation group was lower than those in the control group, the difference was statistically significant(P<0.05). Within 3 months after operation, the incidence of postoperative complication in the observation group was lower than that in the control group, the difference was statistically significant(P<0.05). Conclusion Arthroscopic minimally invasive therapy can improve knee joint function, relieve pain and reduce postoperative complications in patients with tibial plateau fractures.
