北京铁路辖区职工健康体检重要异常结果检后随访研究

摘　　要：目的:通过对北京铁路辖区职工健康体检重要异常结果检后情况进行分析,以制定职工重要异常结果针对性的体检后随访措施,真正做到健康危险因素早发现、早干预、早诊治。方法:回顾性分析北京铁路辖区职工于2016-2019年在北京铁路体检中心健康体检的资料,对其中的重要异常结果进行整理,根据《健康体检重要异常结果管理专家共识(试行版)》中重要异常结果管理规范,针对B类重要异常结果中的超声、放射线、宫颈液基薄层细胞学检查(TCT)、肿瘤标志物等结果进行分析。结果:2016-2019年总参检人数289487例,其中重要异常结果检出率分别为9.34%、7.94%、8.94%和8.56%。2016-2019年共发现恶性肿瘤192例,按照检出方式分类超声109例,放射线26例,TCT 12例,肿瘤标志物45例;按照病种分类甲状腺癌48例,肺癌33例,肝癌18例,肠癌18例,肾癌17例,宫颈癌13例,前列腺癌12例,乳腺癌11例,膀胱癌10例,白血病4例,食管癌2例,胰腺癌2例,卵巢癌2例,胃癌1例,胆囊癌1例。结论:通过体检后随访,健康管理护士对体检职工进行健康宣传、健康维护,讲解相关疾病知识,尽早对健康危险因素进行识别,提高对健康知识的普及率,对防癌工作加大宣传,从而探索重要异常结果的有效管理途径和方式。Objective: Through the analysis on the post-examination state of important abnormal results of health examination of employees in Beijing railway area, formulate targeted post-examination follow-up measures for important abnormal results of employees, so as to truly realize the early detection, early intervention, early diagnosis and early treatment of health risk factors. Methods: The physical examination data of employees under the jurisdiction of Beijing Railway in Beijing Railway Physical Examination Center from 2016 to 2019 were analyzed retrospectively, and the important abnormal results were sorted out. According to the management specifications of important abnormal results in the Expert Consensus on the Management of Important Abnormal Results of Physical Examination (Trial Version), the ultrasound, radiation, liquid-based thinprep cytology test (TCT), tumor markers and other B-level important abnormal results were analyzed. Results: From 2016 to 2019, the total number of people in examinations was 289 487 cases, and the detection rates of important abnormal results were 9.34%, 7.94%, 8.94% and 8.56% respectively. From 2016 to 2019, a total of 192 cases of malignant tumors were found. According to the classification of detection methods, 109 cases were detected by ultrasound, 26 cases were detected by radiation, 12 cases were detected by TCT and 45 cases were detected by tumor markers. According to the classification of the disease, there were 48 cases of thyroid cancer, 33 cases of lung cancer, 18 cases of liver cancer, 18 cases of colon cancer, 17 cases of renal cancer, 13 cases of cervical cancer, 12 cases of prostate cancer, 11 cases of breast cancer, 10 cases of bladder cancer, 4 cases of leukemia, 2 cases of esophageal cancer, 2 cases of pancreatic cancer, 2 cases of ovarian cancer, 1 case of gastric cancer and 1 case of gallbladder carcinoma.
