256排螺旋CT冠脉成像(CTA)在冠心病诊断中的应用价值

摘　　要：目的 探讨256排螺旋CT冠脉成像(CTA)在冠心病诊断中的应用价值。方法 分析该院2020年1月—2021年6月116例疑似冠心病患者的影像学资料,所有患者均行CTA检查,并在2周内行冠状动脉造影(CAG)检查,以CAG结果为金标准,分析CTA检查效能。结果 CAG检查发现阳性87例中,CTA检查诊断为阳性85例,阴性2例;CAG检查发现阴性29例中,CTA检查诊断为阳性1例,阴性28例。以CAG检查结果为金标准,CTA诊断冠心病的灵敏度为97.70%(85/87),特异度为96.55%(28/29),准确度为97.41%(113/116);CTA检查对轻度冠脉狭窄的检出率为92.31%,检查符合率84.62%,中度狭窄检出率为96.00%,检查符合率92.00%,对重度狭窄与闭塞的检出率与检查符合率均为100.00%。结论CTA诊断冠心病准确性良好,具有较高的临床应用价值。Objective To explore the application value of 256-slice spiral CT coronary angiography(CTA) in the diagnosis of coronary heart disease. Methods The imaging data of 116 patients with suspected coronary heart disease in the hospital from January2020 to June 2021 were analyzed. All patients underwent CTA examination and coronary angiography(CAG) examination within 2 weeks. The CAG results were used as the gold standard. Analyzed the performance of CTA examinations. Results 87 cases were found to be positive by CAG examination, of which 85 were positive and 2 were negative by CTA;29 were found to be negative by CAG, of which 1 was positive and 28 were negative by CTA. Taking the results of CAG examination as the gold standard, the sensitivity of CTA in diagnosing coronary heart disease was 97.70%(85/87), the specificity was 96.55%(28/29),and the accuracy was 97.41%(113/116). The detection rate of CTA for mild coronary stenosis was 92.31%, the coincidence rate was 84.62%, the detection rate of moderate stenosis was 96.00%, and the coincidence rate was 92.00%. The detection rate and inspection coincidence rate of severe stenosis and occlusion were both 100.00%. Conclusion CTA has good accuracy in diagnosing coronary heart disease and has high clinical application value.
