加味千金苇茎汤治疗小儿支原体肺炎风热闭肺证

摘　　要：目的:观察加味千金苇茎汤治疗小儿支原体肺炎风热闭肺证的临床疗效,并探讨其对患儿免疫机制的影响。方法:选择符合小儿支原体肺炎诊断标准且中医辨证为风热闭肺证的100例患儿,按照随机对照原则分为对照组和治疗组,每组各50例。其中对照组给予阿奇霉素静脉滴注,治疗组给予阿奇霉素静脉滴注联合加味千金苇茎汤加减,两组均以14 d为1个疗程。两组患儿治疗前后分别观察咳嗽、发热症状消失时间,住院时间,外周血炎症及免疫指标,白细胞介素-6(interleukin-6,IL-6)、IL-10、CD_(3)^(+)/CD_(4)^(+)、CD_(3)^(+)/CD_(8)^(+),临床疗效,并进行评价。结果:两组患儿显效率比较,差异有统计学意义(P<0.05)。与对照组比较,治疗组咳嗽、发热症状消失时间,住院时间明显缩短,差异有统计学意义(P<0.05)。治疗组患儿外周血IL-6、CD_(3)^(+)/CD_(8)^(+)较对照组明显降低,IL-10、CD_(3)^(+)/CD_(4)^(+)明显升高,差异有统计学意义(P<0.05)。结论:加味千金苇茎汤可能通过改善患儿免疫功能及外周血炎症反应,明显缩短患儿咳嗽、发热等临床症状改善时间,提高患儿生活质量。Objective:To observe the clinical efficacy of modified Qianjin Weijing Decoction in the treatment of children with mycoplasma pneumonia with Wind-Heat closed Lung syndrome,and to explore the effect on the immune mechanism of children.Methods:A total of 100 children who met the diagnostic criteria for mycoplasma pneumonia in children and who were diagnosed as Wind-Heat closed Lung syndrome were selected and divided into a control group and a treatment group according to the principle of random control,with 50 cases in each group.The control group was given azithromycin intravenous drip,and the treatment group was given azithromycin intravenous drip combined with modified Qianjin Weijing Decoction.Both groups were given 14 d as a course of treatment.Before and after treatment,the cough,fever symptom disappearance time,hospitalization time,peripheral blood inflammation and immune indexes interleukin-6(interleukin-6,IL-6),IL-10,CD_(3)^(+)/CD_(4)^(+),CD_(3)^(+)/CD_(8)^(+),clinical efficacy and evaluation were observed in the two groups respectively.Results:There was a statistically significant difference between the two groups in the markedly effective rate(P<0.05).Compared with the control group,the disappearance time of cough and fever symptoms and the length of hospital stay in the treatment group were significantly shortened,and the difference was statistically significant(P<0.05).The peripheral blood IL-6 and CD_(3)^(+)/CD_(8)^(+) of the children in the treatment group were significantly lower than those in the control group,while the IL-10 and CD_(3)^(+)/CD_(4)^(+) were significantly increased,and the differences were statistically significant(P<0.05).Conclusion:Modified Qianjin Weijing Decoction may significantly shorten the improvement time of clinical symptoms such as cough and fever,and improve the quality of life of children by improving the immune function and peripheral blood inflammatory response of children.
