摘　　要：目的探讨气体对比剂对于胃癌患者CT成像质量的应用价值。方法纳入303例胃癌患者,根据使用的对比剂将其分为气充盈组143例和水充盈组160例。由2名放射科医师独自评估患者轴位CT图像胃扩张充盈程度,对其进行主观评分分级,并比较2组患者图像质量评级。CT轴位测量胃底、胃体、胃窦正常胃壁厚度,比较2组患者正常胃壁厚度(<5 mm)。结果2名医师对水充盈组CT图像质量评级的一致性检验Kappa值为0.71,气充盈组为0.78。气充盈组轴位CT图像质量评级明显优于水充盈组(Z=-8.979,P<0.001);2组胃底、胃体及胃窦正常胃壁厚度(<5 mm)比较,差异具有统计学意义(P<0.001)。结论相对于水对比剂,气体对比剂充盈胃腔图像质量更佳,尤其对胃窦充盈效果优势更明显,可广泛应用于临床。Objective To investigate the application value of gas contrast agent in CT imaging quality of patients with gastric cancer.Methods A total of 303 patients with gastric cancer were included and divided into the gas filling group with 143 cases and the water filling group with 160 cases according to the used contrast agents.Two radiologists assessed the degree of gastric dilatation and filling in axial CT images of patients independently,made the subjective scores and grades,and the image quality ratings of patients in the two groups were compared.The normal gastric wall thicknesses of gastric fundus,gastric body,and gastric antrum were measured,respectively,and the normal gastric wall thickness(<5 mm)in the two groups was compared.Results The Kappa value of the consistency test of CT image quality ratings by two radiologists was 0.71 in the water filling group and 0.78 in the gas filling group.The axial CT image quality rating of the gas filling group was significantly better than that of the water filling group(Z=-8.979,P<0.001).There were statistically significant differences in the gastric wall thickness(<5 mm)in gastric fundus,gastric body,and gastric antrum(P<0.001).Conclusion Compared with water contrast agent,gas contrast agent has better image quality on filling the gastric cavity,especially for gastric antrum filling,which can be widely used in clinical practices.
