硬膜外阻滞联合射频热凝术治疗颈源性头痛的临床观察与分析

摘　　要：目的观察分析硬膜外阻滞联合射频热凝术治疗颈源性头痛的临床效果。方法选取2018年3月至2020年3月河南省新乡市第一人民医院共收治颈源性头痛住院患者60例,将所有患者按数字表法随机分成两组,观察组30例,对照组30例。对照组患者采用硬膜外注射药物阻滞治疗。观察组患者在此治疗上加入射频热凝术治疗颈源性头痛。结果按照视觉模拟疼痛评分(VAS),偏头痛伤残问卷(MIDAS)评分标准,对照组与观察组相比治疗前VAS评分,差异无统计学意义(P>0.05);对比两组患者,治疗后1周、2周、1个月、3个月、6个月进行VAS评分比较,观察组明显低于对照组,差异有统计学意义(P<0.05);观察组与对照组治疗后1个月、3个月及6个月,MIDAS残疾评级比较,观察组分级明显低于对照组,差异有统计学意义(P<0.05)。结论硬膜外腔阻滞联合射频热凝术治疗颈源性头痛可以降低VAS疼痛评分和MIDAS残疾评级,显著改善疼痛症状,且具有安全、微创、无毒副作用等优点。Objective To observe and analyze the clinical effect of Epidural block combined with Radiofrequency thermocoagulation in the treatment of Cervicogenic headache.Methods A total of 60 inpatients with cervicogenic headache admitted to the First People's Hospital of Xinxiang city,Henan Province from March 2018 to March 2020 were selected.All patients were randomly divided into two groups according to the number table method,30 patients in the observation group and 30 patients in the control group.Control group was treated with epidural drug block.In the observation group,radiofrequency thermocoagulation was added to the treatment for cervical headache.Results Before treatment,there were no significant differences in visual analog pain scale(VAS)and migraine disability Questionnaire(MIDAS)scores between the control group and the observation group(P>0.05);VAS scores of 1 week,2 weeks,1 month,3 months and 6 months after treatment were significantly lower in the observation group than in the control group,with statistical significance(P<0.05).MIDAS disability rating of the observation group was significantly lower than that of the control group at 1,3 and 6 months after treatment,with statistical significance(P<0.05).Conclusion Epidural block combined with radiofrequency thermocoagulation can reduce VAS pain score and MIDAS disability rating,significantly improve pain symptoms,and has the advantages of safety,minimally invasive,and no side effects.
