双氧水冲洗辅助治疗慢性脓胸术中广泛渗血的疗效分析

摘　　要：目的探讨双氧水冲洗用于慢性脓胸术中广泛渗血的治疗效果。方法收集2014年至2019年安徽省胸科医院行胸膜纤维板剥脱术的151例慢性脓胸患者的临床资料。根据有无使用双氧水胸腔冲洗分为双氧水辅助治疗组(83例)和常规对照组(68例),在手术时间、术中出血量、术后胸腔渗出量、术后输血量、术后止血药物使用量、术后胸管置留时间及术后相关并发症等方面相比较,观察有无差异。结果双氧水辅助治疗组相对于常规对照组,在手术时间、术中出血量等方面差异无统计学意义,在术后胸腔引流量、术后拔管时间、术后止血药物使用量、术后输血量等方面均优于对照组,差异有统计学意义(P<0.05),而并未增加气体栓塞发生率。结论双氧水冲洗对慢性脓胸术中广泛渗血的辅助治疗具有很好的临床效果。Objective To investigate the curative effect of hydrogen peroxide irrigation for extensive blood infiltration in chronic empyema surgery.Methods The Clinical data of 151 patients with chronic empyema who underwent pleural fiberboard stripping in our department from 2014 to 2019 were collected.The patients were divided into hydrogen peroxide adjuvant treatment group(83 cases)and routine control group(68 cases)according to the use of hydrogen peroxide thoracic irrigation.The differences were observed in operation time,intraoperative bleeding,postoperative thoracic exudation,postoperative blood transfusion,postoperative hemostatic drug use,postoperative chest tube retention time and postoperative related complications.Results Compared with the conventional control group,the hydrogen peroxide adjuvant treatment group had no significant difference in operation time and intraoperative bleeding.It was better than the control group in postoperative thoracic drainage flow,postoperative extubation time,postoperative hemostatic drug use and postoperative blood transfusion(P<0.05),but did not increase the incidence of gas embolism.Conclusion The application of hydrogen peroxide irrigation in the treatment of extensive blood infiltration in chronic empyema surgery has a good clinical effect.
