摘  要:目的超声造影联合实时弹性成像鉴别浅表肿大淋巴结良恶性的应用价值。方法选取2018年2月至2019年2月在巩义市人民医院超声检查的154例(200枚)浅表肿大淋巴结节患者作为研究对象。所有患者均进行超声造影、实时弹性成像检查,以病理检查结果作为金标准,将检查结果与病理结果作对比,比较良恶性结节的诊断正确率、相关指标及诊断效能。结果200枚肿大淋巴结中,良性淋巴结83枚(41.50%),恶性淋巴结117枚(58.50%);超声造影诊断灵敏度为90.59%、特异度为85.54%、准确度为88.50%;实时弹性成像诊断灵敏度为92.30%、特异度为89.16%、准确度为91.00%;超声造影联合实时弹性成像诊断灵敏度为98.29%、特异度为97.59%、准确度为98.00%;超声造影联合实时弹性成像诊断诊断效能优于单一诊断,差异有统计学意义(P<0.05)。恶性淋巴结淋巴结门髓质主干动脉收缩期血流速度(Vmax)值、阻力指数(RI)值均高于良性淋巴结,舒张期末血流速度(Vmin)值、长短径比(L/S)值均低于良性淋巴结,与良性浅表肿大淋巴结良性病变比较差异有统计学意义(P<0.05)。结论超声造影联合实时弹性成像能有效鉴别浅表肿大淋巴结良恶性,联合使用可提高诊断效能。Objective To evaluate the application value of contrast-enhanced ultrasound combined with real-time elastography in differentiating benign and malignant superficial enlarged lymph nodes.Methods A total of 154 patients(200 lymphonodi)patients with superficial enlarged lymph nodes who were underwent ultrasonography in our hospital from February 2018 to February 2019 were selected as the research subjects.All patients underwent contrast-enhanced ultrasonography and realtime elastography.Using the results of the pathological examination as the gold standard,the examination results were compared with the pathological results to compare the diagnostic accuracy,related indicators and diagnostic efficacy of benign and malignant nodules.Results Among the 200 enlarged lymph nodes,83(41.50%)were benign and 117(58.50%)were malignant;CEUS diagnostic sensitivity was 90.59%,specificity was 85.54%,and accuracy was 88.50%;real-time elastography diagnosis sensitivity was 92.30%,the specificity was 89.16%,and the accuracy was 91.00%;the diagnostic sensitivity of CEUS combined with realtime elastography was 98.29%,the specificity was 97.59%,and the accuracy was 98.00%;the diagnostic performance of CEUS combined with real-time elastography significally better than single diagnosis(P<0.05).Malignant lymph nodes Vmax(lymph node medullary main artery systolic blood flow velocity)value,RI(resistance index)value were higher than benign lymph node,Vmin(end-diastolic blood flow velocity)value,L/S(length-to-short diameter ratio)value were all lower than that of benign lymph nodes,and the difference was statistically significant compared with benign lesions of benign superficial enlarged lymph nodes(P<0.05).Conclusion CEUS combined with real-time elastography can effectively differentiate benign and malignant superficial enlarged lymph nodes,and the combined use can improve the diagnostic efficiency.
