NGAL及炎症细胞因子在2型糖尿病肾脏疾病患者体内的变化情况分析

摘　　要：目的分析中性粒细胞明胶酶相关脂质运载蛋白(NGAL)及炎症细胞因子在2型糖尿病肾脏疾病患者体内的变化情况。方法选取2019年2月至2020年1月收治的2型糖尿病肾病患者150例,根据尿微量白蛋白(UAER)排泄率将患者分为正常白蛋白尿组(UAER<20μg/min)(68例)、微量白蛋白尿组(20μg/min<UAER<200μg/min)(52例)及大量白蛋白尿组(UAER>200μg/min)(30例),选取同期健康体检者50例作为对照组。对所有受检者的NGAL、炎症细胞因子[白细胞介素-6(Interleukin-6,IL-6)和肿瘤坏死因子α(Tumor necrosis factorα,TNF-α)]指标进行检测和比较。结果对照组、正常白蛋白尿组、微量白蛋白尿组及大量白蛋白尿组中的IL-6和TNF-α水平呈明显递增趋势,比较有统计学差异(P<0.05)。对照组、正常白蛋白尿组、微量白蛋白尿组及大量白蛋白尿组的NGAL水平呈明显递增趋势,比较有统计学差异(P<0.05)。结论NGAL、炎症细胞因子的水平会随2型糖尿病肾病患者的病情加重而升高,可作为判断2型糖尿病肾病病情严重程度的敏感指标。Objective To analyze the changes of neutrophil gelatinase-associated lipid carrier protein(NGAL)and inflammatory cytokines in patients with type 2 diabetic kidney disease.Methods A total of 150 patients with type 2 diabetic nephropathy admitted from February 2019 to January 2020 were selected.According to urinary microalbumin excretion rate,patients were divided into normal albuminuria group(UAER<20μg/min)(68 cases),microalbuminuria group(20μg/min<UAER<200μg/min)(52 cases)and macroalbuminuria group(UAER>200μg/min)(30 cases).Fifty healthy subjects were selected as the control group.NGAL,INTERleukin-6(IL-6),and Tumor necrosis factorα(TNF-α)levels were measured and compared in all subjects.Results The levels of IL-6 and TNF-αin control group,normoalbuminuria group,microalbuminuria group and macroalbuminuria group were significantly increased,and there were statistical differences(P<0.05).The NGAL levels of control group,normoalbuminuria group,microalbuminuria group and macroalbuminuria group showed a significant increasing trend,with statistical differences(P<0.05).Conclusion The levels of NGAL and inflammatory cytokines will increase with the aggravation of type 2 diabetic nephropathy,which can be used as sensitive indicators to judge the severity of type 2 diabetic nephropathy..
