腹腔镜直肠癌根治术中行无瘤护理的效果分析

摘　　要：目的:研究腹腔镜直肠癌根治术中行无瘤护理的临床效果。方法:选取2018年1月-2019年1月怀集县人民医院收治的直肠癌患者88例,按随机数字表法分为对照组和观察组,各44例。对照组予以常规护理,观察组在对照组的基础上予以无瘤护理。观察两组术中情况、手术指标、复发率、转移率及死亡率情况。结果:观察组术中出血量少于对照组、手术时间长于对照组,差异有统计学意义(P<0.05);两组淋巴清扫枚数比较,差异无统计学意义(P>0.05);观察组胃肠功能恢复时间、导管留置时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。随访期间,观察组患者复发率、转移率及病死率均低于对照组患者,差异有统计学意义(P<0.05)。结论:腹腔镜直肠癌根治术中行无瘤护理可以缩短住院时间,减少术中出血,利于术后恢复,减少患者肿瘤的复发和转移。Objective:To study the clinical effect of tumor-free nursing in laparoscopic radical resection of rectal cancer.Methods:A total of 88 patients with rectal cancer admitted to Huaiji County People's Hospital from January 2018 to January 2019 were selected and divided into a control group and an observation group according to a random number table,with 44 cases in each group.The control group was given routine nursing care,and the observation group was given tumor-free nursing on the basis of the control group.The intraoperative conditions,surgical indicators,recurrence rate,metastasis rate and mortality rate of the two groups were observed.Results:The intraoperative blood loss of the observation group was less than that of the control group,and the operation time was longer than that of the control group,with statistically significant difference(P<0.05);there was no statistically significant difference in the number of lymphatic node dissections between two groups(P>0.05);the gastrointestinal function recovery time,catheter indwelling time,and hospitalization time in the observation group were shorter than those in the control group,with statistically significant difference(P<0.05).During the follow-up period,the recurrence rate,metastasis rate and mortality rate of patients in the observation group were lower than those in the control group,with statistically significant difference(P<0.05).Conclusion:Tumor-free nursing during laparoscopic radical resection of rectal cancer can shorten the hospitalization time,reduce intraoperative blood loss,facilitate postoperative recovery,and reduce the recurrence and metastasis of tumor.
