摘　　要：目的:探讨对糖尿病患者采用社区医护整合门诊方法实施血糖管理后获得临床效果。方法:选取2017年1月2018年12月上海市长宁区新泾镇社区卫生服务中心收治的200例2型糖尿病患者作为研究对象;随机分为对照组进行常规干预和试验组进行社区糖尿病专管护士+家庭医生整合门诊的方式干预,每组各100例。比较两组患者依从性评分、血糖控制情况。结果:试验组依从性评分高于对照组,差异有统计学意义P<0.05;干预前,两组FPG、HbA_(1c)、2 hPG血糖情况比较,差异无统计学意义(P>0.05);干预后,试验组FPG、HbA_(1c)、2 hPG均低于对照组,差异有统计学意义(P<0.05)。结论:社区医护整合门诊方法应用于社区糖尿病患者血糖管理,可显著提高患者依从性评分,显著改善血糖控制情况,提高血糖管理整体预后水平。Objective:To explore the clinical effect of integrated clinic of community nurses and doctors on the implementation of blood glucose management in diabetic patients.Methods:A total of 200 patients with type 2 diabetes from January 2017 to December 2018 in Community Health Service Center of Xinjing Town in Changning District of Shanghai were selected as the subjects;they were randomly divided into control group(routine intervention)and experimental group(intervention of integrated clinic of community diabetes nurses and family doctors),with 100 patients in each group.The compliance score and blood glucose control of patients were compared between the two groups.Results:The compliance score of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Before the intervention,there was no statistically significant difference between the two groups in FPG,HbA1c and 2 h PG(P<0.05);after the intervention,the FPG,HbA1c and 2 h PG in the experimental group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of the integrated clinic of community nurses and doctors in the blood glucose management of community diabetes patients can significantly increase the compliance score,significantly improve blood glucose control,and improve the overall prognosis of blood glucose management.
