摘 要:目的 掌握河北省儿童乙型流感流行特征和维多利亚系(BV)乙型流感病毒血凝素(HA)基因变异情况。方法收集2018—2020年河北省0~14岁儿童流感样病例(ILI)监测数据,并对分离的14株BV毒株的HA基因序列进行分析。结果 河北省儿童ILI就诊百分比(ILI%)平均为2.36%,共检测咽拭子标本18 955份,乙型流感病毒核酸阳性439份,检出率为2.32%,其中BV占94.53%(415/439)。不同年龄组病例间检出率差异有统计学意义(χ^(2)=93.357,P<0.001),学龄组最高(3.39%),婴儿组检出率最低(0.56%)。2018—2020年共报告BV流感疫情16起,均发生在小学,春季高发。14株BV流感毒株HA基因均属于162~164位氨基酸缺失株,与疫苗株相比涉及到10个氨基酸位点突变,不同地域毒株有独特的变异位点。结论 2018—2020年河北省儿童乙型流感冬春季流行,以BV为主,学龄儿童高发,HA基因发生了个别位点的变异。Objective To understand the epidemiological characteristics of influenza B in children in Hebei province,and to analyze the variation of hemagglutinin(HA) gene of influenza B-Victoria(BV) strains. Methods The analysis was conducted on the data of cases of influenza like illness(ILI) in children aged 0-14 years old in Hebei province from 2018 to 2020. The HA gene sequences of 14 influenza B-Victoria strains were analyzed. Results The average visit percentage of ILI of children in Hebei province was 2.36%. During the period,a total of 18 955 throat swab samples were detected,and 439 influenza B virus nucleic acid positive samples were confirmed,with the detection rate of 2.32%. BV accounted for 94.53%(415/439). The positive rate showed a statistical significance in different age groups(χ2=93.357,P<0.001),the highest positive rate(3.39%) was in school age group,while the lowest(0.56%) was in infant group. A total of 16 epidemics of BV influenza were reported from 2018 to 2020,all of which occurred in primary school children with high incidence in spring. The HA genes of the 14 BV influenza strains belonged to the amino acid deletion strains at 162-164 sites. Compared with the vaccine strains,the 10 amino acid mutations were involved,and the strains from different regions had unique mutation position. Conclusion Influenza B among children was prevalent in winter and springin Hebei province from 2018 to 2020,and BV viruses was predominant. The infections were intensive in school-age children and the HA gene was mutated at individual positions.
