摘 要:目的 检测临床分离的广泛耐药肺炎克雷伯菌菌株的16S rRNA甲基化酶基因。方法 2019年2月—2020年12月延安大学附属医院分离的广泛耐药肺炎克雷伯菌59株,行菌株对4种抗生素的药敏试验,用PCR方法检测6种16S rRNA甲基化酶基因。结果 肺炎克雷伯菌对阿米卡星、庆大霉素、妥布霉素、奈替米星的耐药率分别为88.14%、98.31%、89.83%、91.53%,对庆大霉素的耐药率高于阿米卡星、奈替米星,差异有统计学意义(P<0.05)。16S rRNA甲基化酶基因检出率为66.10%,其中armA、rmtB基因检出率分别为23.73%、42.37%,未检出其他4种甲基化酶基因。结论16S rRNA甲基化酶基因在耐药肺炎克雷伯菌分离株中广泛分布,以armA、rmtB基因为主。Objective To detect the 16 S rRNA methylase gene of Klebsiella pneumoniae. Methods A total of 59 strains of extensively resistant Klebsiella pneumoniae clinically isolated in Yan’an University Affiliated Hospital were collected from February 2019 to December 2020. Drug sensitivity test of strains to four antibiotics and 6 kinds of16 S rRNA methylase gene test were performed by PCR. Results The resistance rates of Klebsiella pneumoniae strains to amikacin,gentamicin,tobramycin and netilmicin was 88.14%,98.31%,89.83% and 91.53%,respectively. The resistance rate of Klebsiella pneumoniae strains to gentamicin was higher than amikacin and netilmicin(P<0.05). The total detection rate of 16 S rRNA methylase genes was 66.10%,among which the detection rates of armA and rmtB genes were 23.73% and 42.37%,respectively,no other 4 kinds of methylase gene were detected. Conclusion 16 S rRNA methylase gene is widely distributed in drug-resistant Klebsiella pneumoniae isolates,mainly armA and rmtB genes.
