睾酮替代治疗慢性耳鸣伴迟发性性腺功能减退症的疗效分析

摘　　要：目的探讨慢性耳鸣伴迟发性性腺功能减退症(LOH)患者应用睾酮替代治疗的疗效及价值。方法选择2019年9月至2021年1月在牡丹江医学院附属红旗医院耳鼻喉科、内分泌科及泌尿外科门诊就诊的伴发慢性耳鸣的LOH男性患者60例,按照临床就医时间先后顺序将入选患者分为试验组及对照组,每组各30例;对照组给予银杏叶软胶囊治疗,试验组于对照组治疗基础上增加十一酸睾酮胶囊治疗,两组均治疗3个月后,比较两组患者的耳鸣疗效,疗效评估采用耳鸣致残量表(THI)评分。结果治疗后试验组TT值较对照组提高(P<0.05),两组治疗后THI评分均较治疗前降低(P<0.05),治疗后试验组THI评分相较于对照组更低(P<0.05)。治疗后试验组耳鸣总有效率高于对照组,两组间疗效比较,差异有统计学意义(P<0.05)。结论在慢性耳鸣伴LOH患者的治疗中,对于无耳部器质性病变,传统的耳科治疗方法无效,可考虑采用睾酮替代治疗为主的治疗方案,使患者耳鸣症状得到较好改善。Objective To investigate the effect of testosterone replacement therapy on tinnitus in patients with chronic tinnitus and late-onset hypogonadism.Methods A total of 60 male LOH patients with chronic tinnitus who visited the ENT,Endocrinology and Urology Outpatient Departments of Hongqi Hospital Affiliated to Mudanjiang Medical College from September 2019 to January 2021 were selected and divided into the experimental group(n=30)and the control group(n=30)according to the order of clinical treatment.The control group was treated with Ginkgo leaves soft-gel capsule.Testosterone undecanoate capsule was added to the treatment in the experirmental group.After treatment for 3 months,the tinnitus efficacy was compared between the two groups.The tinnitus handicap inventory(THI)score was used to evaluate the efficacy.Results After treatment,the TT value of the experimental group was higher than that of the control group(P<0.05).The THI score of the two groups after treatment was lower than that before treatment(P<0.05).After treatment,the THI score of the experimental group was lower than that of the control group(P<0.05).After treatment,the total effective rate of tinnitus in the experimental group was higher than that in the control group,and the difference in the efficacy between the two groups had statistical significance(P<0.05).Conclusion In the treatment of patients with chronic tinnitus accompanied by LOH,the traditional otological treatment is ineffective for patients without ear organic disease,and testosterone replacement therapy-based treatment can be considered to improve the tinnitus symptoms of patients.
