指法按摩会阴体对初产妇分娩会阴撕裂伤程度的影响

摘　　要：目的 研究指法按摩会阴体运用于初产妇分娩中的价值。方法 选择2019年8月至2020年5月江西省萍乡市妇幼保健院纳入的初产妇100例,按随机数字表法分为三组,其中观察组40例采用第二产程改良药物会阴按摩,对照1组30例不行会阴按摩,仅提供阿托品及利多卡因湿敷,对照2组30例不行会阴按摩、阿托品及利多卡因湿敷,常规助产,对三组会阴撕裂程度进行评价,统计三组产后2 h与24 h失血量、住院天数,并进行视觉模拟评分法(VAS)评分,统计三组的会阴侧切率,比较各组的干预结果。结果 观察组会阴撕裂程度轻于对照1、2组(P<0.05)。观察组产后2 h与24 h失血量、住院天数、VAS评分均低于对照1、2组(P<0.05)。观察组会阴侧切率为22.50%,低于对照1、2组的46.67%、60.00%(P<0.05)。结论 指法按摩会阴体在初产妇分娩中效果显著,能够有效减轻会阴撕裂程度,减少失血量及住院天数,减低会阴侧切率,值得推广。Objective To study the value of massage on the perineal body in primipara delivery. Methods A total of100 primiparas included in Pingxiang Maternity and Child Health Hospital in Jiangxi Province from August 2019 to May 2020 were selected and divided into three groups according to the random number table method. Among them, 40patients in the observation group received modified perineal massage for the second stage of labor. Thirty patients in control group 1 were not given perineal massage, and only provided with atropine and lidocaine wet compresses. Thirty cases of control group 2 were not given perineal massage, atropine, and lidocaine wet compresses. They were provided with routine birth assistance. The degree of perineal laceration in the three groups was evaluated. Blood loss at 2 h and24 h after delivery, and the hospital days in the three groups were counted. The visual analog scale(VAS) scores were performed. The perineal lateral resection rates among the three groups were calculated. The intervention results among groups were compared. Results The degree of perineal laceration in the observation group was lighter than that in the control group 1 and 2(P<0.05). The blood loss at 2 h and 24 h after delivery, hospitalization days, and VAS scores in the observation group were lower than those in the control group 1 and 2(P<0.05). The perineal lateral resection rate in the observation group was 22.50%, which was lower than that in the control group 1 and 2(46.67%, 60.00%)(P<0.05). Conclusion Massage on the perineal body is effective in the delivery of primiparas. It can effectively minimize the degree of perineal laceration, reduce blood loss and hospital days, and reduce the rate of lateral perineal resection.It is worthy of promotion.
