摘　　要：目的分析脑电双频谱指数(BIS)监测对急性呼吸窘迫综合征(ARDS)机械通气患者镇静效果的影响。方法本研究共80例研究对象,均为2021年1~4月来赣南医学院第一附属医院进行有创机械通气治疗的ARDS患者,采用随机数字表法分为对照组(RASS评分,n=40)与试验组(BIS监测,n=40),两组均采用常规咪达唑仑注射液或丙泊酚注射液进行镇静处理。对比分析两组的镇静效果。结果试验组镇静良好率高于对照组(P<0.05)。在试验组的持续镇静时长、机械通气天数、住院天数上短于对照组(P<0.05)。试验组的不良事件发生率低于对照组(P<0.05)。试验组的心率(HR)、中心静脉压(CAP)及平均动脉压(MAP)明显低于对照组(P<0.05)。结论BIS监测用于ARDS机械通气患者的效果理想,镇静效果良好,有利于调节患者的血流动力学状态,改善患者的预后,且不良反应少,临床可进一步推广运用。Objective To analyze the effect of bispectral index(BIS)monitoring on sedation in patients with acute respiratory distress syndrome(ARDS)undergoing mechanical ventilation.Methods A total of 80 patients with ARDS who received invasive mechanical ventilation in our hospital from January to April 2021 were selected in this study.They were randomly divided into the control group(RASS score,n=40)and the experimental group(BIS monitoring,n=40).Both groups were sedated with conventional midazolam injection or propofol injection.The sedative effects of the two groups were compared and analyzed.Results The rate of good sedation in the experimental group was higher than that in the control group(P<0.05).The duration of continuous sedation,mechanical ventilation and hospitalization in the experimental group were shorter than those in the control group(P<0.05).The incidence of adverse events in the experimental group was lower than that in the control group(P<0.05).The heart rate(HR),central venous pressure(CAP)and mean arterial pressure(MAP)in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion BIS monitoring shows ideal effect on ARDS patients with mechanical ventilation,which has good sedative effect,and it is helpful to regulate the hemodynamic state of patients,improve the prognosis of patients,and less adverse reactions,so it can be further popularized and used in clinic.
