摘 要:目的基于神经递质和下丘脑-垂体-肾上腺轴探讨壮医刺血疗法治疗失眠大鼠的作用机制。方法将40只SD大鼠随机分为空白组、模型组、壮医刺血组、西药组,每组10只。除空白组外,其他组均采用腹腔注射对氯苯丙氨酸建立失眠模型(共2天)。建模成功后连续干预12 d,其中壮医刺血组采用大鼠背部穴位针刺并放血干预,西药组、模型组分别给予地西泮、生理盐水灌胃,而空白组给予抓取刺激干预。记录治疗前后各组大鼠的体重。于干预结束后次日,检测大鼠血清皮质醇、促肾上腺皮质激素(ACTH)含量,以及下丘脑5-羟色胺、去甲肾上腺素(NE)、促肾上腺皮质激素释放激素(CRH)含量。结果与空白组相比,模型组大鼠下丘脑组织5-羟色胺含量降低,下丘脑组织NE、CRH及血清皮质醇、ACTH含量均升高(均P<0.05)。与模型组相比,西药组、壮医刺血组大鼠下丘脑组织5-羟色胺含量升高,壮医刺血组和西药组大鼠下丘脑组织NE、CRH和血清皮质醇、ACTH含量均下降(均P<0.05)。与西药组相比,壮医刺血组下丘脑组织CRH含量、血清皮质醇含量均降低(均P<0.05)。结论壮医刺血疗法可通过调节5-羟色胺和下丘脑-垂体-肾上腺轴相关激素而改善对氯苯丙氨酸诱导的大鼠失眠状态。Objective To investigate the mechanism of action of traditional Zhuang medicine acupuncture and bloodletting therapy in the treatment of insomnia rats based on the neurotransmitters and hypothalamic-pituitary-adrenal(HPA)axis.Methods Forty SD rats were randomly divided into blank group,model group,traditional Zhuang medicine acupuncture and bloodletting group and Western medicine group,with 10 rats in each group.Except for the blank group,the other groups received intraperitoneal injections of para-chlorophenylalanine(PCPA)to establish the models of insomnia(totaling two days).The intervention was performed for 12 consecutive days after the successful modeling,wherein rats in the traditional Zhuang medicine acupuncture and bloodletting group underwent acupuncture and bloodletting at acupoints on the backs;the Western medicine group and the model group were given diazepam and normal saline intragastrically,respectively;and the blank group was intervened with grasping stimulation.The body weights of rats before and after the treatment were recorded in each group.On the next day after the end of the intervention,a detection was conducted on the rats for serum contents of cortisol and adrenocorticotropic hormone(ACTH),as well as the hypothalamic contents of 5-hydroxytryptamine(5-HT),norepinephrine(NE),and corticotropin-releasing hormone(CRH).Results Compared with those in the blank group,rats in the model group exhibited a lower content of 5-HT in hypothalamic tissues,higher contents of NE and CRH in hypothalamic tissues,as well as elevated serum contents of cortisol and ACTH(all P<0.05).Compared with those in the model group,rats in the Western medicine and traditional Zhuang medicine acupuncture and bloodletting groups yielded higher contents of 5-HT in hypothalamic tissues,lower contents of NE and CRH in hypothalamic tissues,as well as decreased serum contents of cortisol and ACTH(all P<0.05).The traditional Zhuang medicine acupuncture and bloodletting group had lower contents of hypothalamic CRH and serum cortiso
