摘  要:目的 评价综合评估法在牙体牙髓模拟教学中的应用效果。方法 选取2020年6—10月在本院口腔医学专业实习的2016级实习培训生16名,使用下颌第一磨牙仿真牙髓树脂牙进行模拟开髓操作,通过教师评分并指导学生完成自评分,并逐一反馈评分结果。8个周后再次进行如上操作和评分,通过比较两次模拟操作评分差异,来评价教学效果。结果 相对于第一次评分,学生第二次开髓操作时在操作程序、髓室顶去净、根管口暴露这三个细化评分项上有明显进步(P<0.05),而在对开口位置及洞形的把握以及保持髓室底壁完整方面评分差异无统计学意义(P> 0.05)。从评分成绩上分析,两次临床模拟操作中,学生的自我评估与教师评估的差异均无统计学意义(P>0.05)。总体而言,两次临床模型操作中,学生自评分和教师评分一致性低(Kappa值<0.4),但细化评分而言,在第一次模拟临床操作中,学生自评分和教师评分在操作程序、开口位置及洞形和髓室顶去净方面上的一致性尚可(0.4≤Kappa值<0.75),而在第二次模拟临床操作中,除去根管口暴露评分项目的一致性尚可外,其余项目合格分级评分一致性均低(Kappa值<0.4)。结论 在牙体牙髓模拟教学中强调自我评估能力培训及个体化反馈的综合评估教学法,具有良好的教学效果,为日后的临床实践奠定坚实基础。Objective To evaluate the application effect of the comprehensive evaluation method in simulation teaching of Endodontics.Methods 16 trainees of grade 2016 who practiced in the Department of stomatology in our hospital from June to October 2020 were selected.The simulated pulp opening operation was carried out by using the simulated pulp resin teeth of the mandibular first molar.Through the teacher’s scoring and guiding the students to complete the self scoring,the scoring results were fed back one by one.After 8 weeks,the above operation and scoring were carried out again.The teaching effect was evaluated by comparing the scoring differences of the two simulated operations.Results Compared with the first score,the students made significant progress in the three detailed scores of operation procedure,cleaning of pulp chamber top and exposure of root canal orifice during the second pulpotomy(P<0.05),but there was no significant difference in the grasp of opening position and hole shape and the integrity of pulp chamber bottom wall(P>0.05).From the score analysis,there was no significant difference between students’ self-assessment and teachers’ assessment in the two clinical simulation operations(P>0.05).In general,the consistency between student self score and teacher score was low in the two clinical model operations(Kappa value<0.4),but in terms of detailed score,in the first simulated clinical operation,the consistency between student self score and teacher score in operation procedure,opening position,hole shape and removal of medullary chamber top was fair(0.4 ≤ Kappa value<0.75),while in the second simulated clinical operation.Except for the consistency of root canal exposure score items,the consistency of other items was low(Kappa value<0.4).Conclusion Comprehensive evaluation method that emphasizes self-evaluation ability training and individualized feedback has a good teaching effect in endodontics,and lays a solid foundation for future clinical practice.
