摘  要:目的观察达格列净对2型糖尿病合并高尿酸血症患者血尿电解质及尿酸的影响。方法收集2型糖尿病合并高尿酸血症患者120例,随机分为对照组及观察组,两组患者均给予盐酸二甲双胍片,观察组在此基础上加用达格列净,随访观察两组患者12周、24周的情况,检测血清电解质、糖化血红蛋白及尿酸;检测尿电解质、尿微量蛋白、24 h尿尿酸。结果经过24周治疗,试验期间两组血电解质(血钠、血钾、血钙)随时间的变化趋势无统计学意义(P>0.05);24 h尿电解质(尿钠、尿钾、尿钙)随时间的变化趋势具有显著统计学意义(P<0.01);12周时观察组患者血尿酸与初诊时比较,差异有统计学意义(P<0.05);24周时,观察组24 h尿尿酸与初诊时比较,差异有统计学意义(P<0.05),观察组与对照组比较24 h尿尿酸均升高,差异有统计学意义(P<0.05)。结论达格列净能够显著降低2型糖尿病合并高尿酸血症患者的糖化血红蛋白及血尿酸,升高24 h尿尿酸,无明显增加尿量,虽然能增加24 h尿电解质,但并不影响血电解质水平。Objective To observe the effect of dagglitazine on blood and urine electrolytes and urine acids in patients with type 2 diabetes mellitus and hyperuricemia.Methods 120 patients with type 2 diabetes mellitus and hyperuricemia were randomly divided into the control group and the observation group.The patients in the two groups were given metformin hydrochloride tablets.On this basis,the observation group was added with dagglitazine.The patients in the two groups were followed up for 12 weeks and 24 weeks.Serum electrolytes,glycosylated hemoglobin and uric acid were measured.Urine electrolytes,urine micro protein and 24 hours urine uric acid were measured.Results After 24 weeks of treatment,there was no significant change trend of blood electrolytes(blood sodium,blood potassium and blood calcium)with time between the two groups(P>0.05);The change trend of 24-hour urinary electrolyte(urinary sodium,urinary potassium and urinary calcium)with time was statistically significant(P<0.01);At 24 weeks,the difference of 24-hour urine uric acid between the observation group and the control group was statistically significant(P<0.05);The difference of 24-hour urine uric acid between the observation group and the control group was statistically significant(P<0.05).Conclusion Dagglitazine can significantly reduce glycosylated hemoglobin and blood uric acid in type 2 diabetes mellitus patients with hyperuricemia,increase 24-hour urine uric acid,without significantly increasing urine volume.Although it can increase 24-hour urine electrolyte,it does not affect blood electrolyte level.
