含服盐酸达克罗宁胶浆后不同时段行超声胃镜对胃癌高风险人群筛查的效果

摘　　要：目的探讨含服盐酸达克罗宁胶浆后不同时段行超声胃镜对胃癌高风险人群筛查效果的影响。方法将佛山市第一人民医院内镜中心2020年8月至2021年5月接收的行超声胃镜检查的胃癌高危人群150例随机分为A组、B组及C组,每组各50例;A组在超声胃镜检查前10 min含服盐酸达克罗宁胶浆10 mL,B组为检查前15 min含服,C组为检查前20 min含服;对比三组的麻醉效果、祛泡效果、检查中血流动力学、检查后的不良反应及检查时间、疼痛评分。结果A、B、C三组患者麻醉优良率分别为94.00%、84.00%、78.00%;A组的麻醉效果显著优于C组(P<0.05);A、B、C三组患者的祛泡优良率分别为78.00%、76.00%、82.00%,三组祛泡效果比较无统计学差异(P>0.05);C组在胃镜通过咽喉(T_(1)),通过贲门(T_(2)),通过幽门(T_(3))的收缩压(SBP)、舒张压(DBP)、心率(HR)均高于A组,C组在T_(1)~T_(3)时刻的SpO_(2)低于A组(P<0.05);B、C组T_(2)~T_(3)时刻的SBP、DBP、HR、SpO_(2)与T0时刻比较均有较大波动(P<0.05);三组患者的不良反应比较无统计学差异(P>0.05);A组患者的检查时间及疼痛评分均低于C组,B组的疼痛评分低于C组(P<0.05)。结论含服盐酸达克罗宁胶浆10 min麻醉效果最佳,祛泡效果并未明显下降,且检查中血流动力学平稳,有助于缩短检查时间,减轻疼痛感。Objective To explore the efficacy of ultrasound gastroscopy at different time intervals after oral administration of dyclonine hydrochloride gel on the screening effect of high-risk groups of gastric cancer.Methods 150 cases high-risk gastric cancer patients undergoing ultrasound gastroscopy received from the Endoscopy Center of the Foshan First People's Hospital from August 2020 to May 2021 were randomly divided into the group A,B and C,50 cases in each group.The Group A were treated with 10mL of dyclonine hydrochloride gel 10 minutes before the ultrasound gastroscopy,the group B 15 minutes before the examination,and the group C 20 minutes before the examination.The anesthesia effects,defoaming effects,Hemodynamics,adverse reactions,inspection time,pain score were compared.Results The excellent and good rates of anesthesia in groups A,B,and C were 94.00%,84.00%and 78.00%,respectively.The anesthesia efficacy of group A were significantly better than that of group C,and the difference were statistically significant(P<0.05).The excellent and good rates of defoaming in Group A,B and C were 78.00%,76.00%and 82.00%,respectively.There were no significant difference in the effect of defoaming in the three groups(P>0.05).The SBP,DBP,and HR of group C through the throat(T_(1)),through the cardia(T_(2)),and through the pylorus(T_(3))were higher than those of group A,while the SpO_(2) of group C were lower than that of group A at T_(1)~T_(3),and the difference were statistically significant(P<0.05).The SBP,DBP,HR,SpO_(2) in the group B and C at the T_(2) to T_(3) compared with T_(0) with statistically significant(P<0.05).The adverse reactions of the three groups of patients were no statistically different(P>0.05).The examination time and pain score of group A were lower than those of group C,and the pain score of group B were lower than that of group C.The difference was statistically significant(P<0.05).Conclusion The effect of oral administration of dyclonine hydrochloride glue for 10 minutes is the best anesthesia,the de
