摘 要:目的探讨影响结肠癌CD55蛋白表达的临床病理因素及其与预后的关系。方法利用TCGA数据库(https://portal.gdc.cancer.gov/)进行生物信息学分析,比较CD55 mRNA表达在结肠癌组织和正常组织中的差异;回顾性分析2014年10月至2015年10月北京大学人民医院收治的109例结肠癌病人的临床病理资料,免疫组织化学方法检测结肠癌组织和癌旁组织CD55蛋白表达,探索影响CD55蛋白表达的临床病理因素,单因素和多因素分析其表达与预后的关系。结果CD55蛋白在正常结肠黏膜组织不表达,而在结肠癌组织的细胞膜表面和细胞质中高表达。109例结肠癌组织中,CD55蛋白高表达者占25.7%(28/109),低表达者占74.3%(81/109)。CD55表达与结肠癌病人性别、年龄、分化程度、肿瘤部位、T分期、N分期、错配修复状态(MMR)无关(P>0.05),而与有无转移(χ^(2)=8.535,P=0.003)和TNM分期(χ^(2)=8.626,P=0.035)相关。分层分析显示,肿瘤部位、CD55蛋白表达、有无远处转移与Ⅲ~Ⅳ期结肠癌病人OS相关;多因素Cox回归分析发现,CD55蛋白表达水平、有无远处转移是影响Ⅲ~Ⅳ期结肠癌病人术后总生存的独立危险因素。结论CD55蛋白表达升高与结肠癌病人不良预后有关,是影响Ⅲ~Ⅳ期结肠癌病人术后总生存的独立危险因素,其可能成为结肠癌病人新的治疗方向。Objective To investigate the clinicopathological factors affecting the expression of CD55 protein in colon cancer and its relationship with prognosis.Methods The TCGA database(https://portal.gdc.cancer.gov/)was used for bioinformatics analysis to explore the difference of CD55 m RNA expression between colon cancer tissues and normal tissues.The clinicopathological data of 109 patients with colon cancer treated in Peking University People’s Hospital from October 2014 to October 2015 were analyzed retrospectively.Immunohistochemistry method was used to detect the expression of CD55 protein in colon cancer and adjacent tissues.The relationship between clinicopathological factors and CD55 expression was analyzed,and the difference in survival between high and low CD55 expression groups was compared by univariate and multivariate cox proportional hazards model.Results CD55 protein was not expressed in normal colon mucosa,but it was highly expressed on the cell membrane surface and cytoplasm of colon cancer.Among the 109 colon cancer tissues,25.7%(28/109)had high expression of CD55 protein,and 74.3%(81/109)had low expression.The expression of CD55 was not correlated with gender,age,degree of differentiation,tumor location,T stage,N stage and mismatch repair status(MMR)of colon cancer patients(P>0.05),but was associated with the metastasis(χ^(2)=8.535,P=0.003)and TNM stage(χ^(2)=8.626,P=0.035).Stratified analysis showed that tumor location,CD55 protein expression,and distant metastasis were associated with OS in patients with stage Ⅲ-Ⅳ colon cancer.Multivariate Cox regression analysis found that CD55 protein expression level and distant metastasis were independent risk factors for postoperative overall survival in patients with stage Ⅲ-Ⅳ colon cancer.Conclusion The increased expression of CD55 protein is related to the poor prognosis of patients with colon cancer.It is an independent risk factor affecting the overall survival time of patients with stage Ⅲ~Ⅳ colon cancer.CD55 may become a new therapeutic
