DAA全髋关节置换术术前准备改良方法的临床应用

摘　　要：目的:评价直接前方入路(DAA)全髋关节置换术术前准备改良方法,探讨其临床推广价值。方法:回顾性分析2020年1月至2020年11月在成都市第二人民医院骨外科完成DAA初次全髋关节置换术54例患者的临床资料,按照是否使用改良方法分为改良组和传统组,对两组患者术前准备时间、手术时间、消毒面积及Harris评分进行比较。结果:两组患者的术前准备时间和消毒面积比较差异有统计学意义(P<0.05);两组患者的手术时间用时较为接近,差异无统计学意义(P>0.05)。两组患者术前Harris评分比较差异无统计学意义(P>0.05),改良组术后Harris评分高于传统组,差异有统计学意义(P<0.05)。结论:改良后DAA能减少术区消毒面积、缩短手术准备时间、减少麻醉时长、降低麻醉风险、有利于患者术后髋关节功能快速康复,具有临床推广应用价值。Objective:To evaluate the improved method of preoperative preparation for direct anterior approach(DAA)total hip arthroplasty so as to explore its clinical value.Methods:From January 2020 to November 2020,the clinical data of 54 patients who completed DAA primary total hip arthroplasty in the Department of Orthopedics of The Second People’s Hospital of Chengdu were retrospectively analyzed.According to whether the modified method was used or not,the patients were divided into the modified group and the traditional group.The preoperative preparation time,operation time,disinfection area and Harris score were compared between the two groups.Results:There were significant differences in preoperative preparation time and disinfection area between the two groups(P<0.05).The operation time of the two groups was similar,and the difference was not statistically significant(P>0.05).There was no significant difference in preoperative Harris score between the two groups(P>0.05).The postoperative Harris score in the modified group was higher than that in the traditional group,with a statistically significant difference(P<0.05).Conclusion:After the improvement,DAA can reduce the disinfection area,shorten the operation preparation time as well as the duration of anesthesia and the risk of anesthesia.And it is conducive to the rapid recovery of the hip joint function of patients after surgery,which has clinical application value.
