摘 要:目的探讨颅脑CT监测在重型颅脑损伤去骨瓣减压术后的应用分析。方法回顾性分析收治的54例颅脑损伤患者的研究资料,患者均进行急诊全麻下颅内血肿清除+去骨瓣减压术,术后分别对所有患者进行颅脑CT和床旁超声监测。分析患者采用不同检监测方法的影像学表现,对比两种监测方法对迟发颅内症状的检出情况。结果颅脑CT和床旁超声对脑挫裂伤、脑梗死、硬膜外血肿、硬膜下血肿及脑内血肿的检出率经比较,差异均无统计学意义(P>0.05),但是颅脑CT的总检出率高于床旁超声(P<0.05)。颅脑CT和床旁超声对血肿前后径、血肿左右径、血肿上下径、出血量、中线移位及侧脑室宽度绝对性值的检测值经比较,差异均无统计学意义(P>0.05)。结论颅脑CT和床旁超声在临床重型颅脑损伤去骨瓣减压术后的监测中均有较好的应用效果及价值,但是颅脑CT对迟发颅内症状的检出率相对更高,床旁超声的检测方法更为便捷,且无创,能够动态进行监测,临床可根据患者情况酌情选择合适的监测方法。Objective To investigate the application of craniocerebral CT monitoring after decompression of craniocerebral craniocerebral injury.Methods 54 patients with traumatic brain injury in our hospital were retrospectively analyzed research data,the patients in emergency department of general down to intracranial hematoma removal and bone disc decompression,respectively,for all of the patients with craniocerebral CT postoperative and bedside ultrasound monitoring.The imaging findings of patients using different monitoring methods were analyzed,and the detection of delayed intracranial symptoms by two monitoring methods was compared.Results There was no significant difference in the detection rate of brain contusion and laceration,cerebral infarction,epidural hematoma,subdural hematoma and intracerebral hematoma between head CT and bedside ultrasound(P<0.05),but the total detection rate of head CT was higher than that of bedside ultrasound(P<0.05).There were no significant differences in the anterior and posterior diameter,left and right diameter,upper and lower diameter of hematoma,blood loss,midline displacement and absolute value of lateral ventricle width between brain CT and bedside ultrasound(P>0.05).Conclusion craniocerebral CT and bedside ultrasound in clinical severe head injury to the bone disc decompression in the postoperative monitoring all have good application effect and value,but the craniocerebral CT detection rate is relatively higher for the delayed intracranial symptoms,bedside ultrasonic detection method is more convenient,and noninvasive,dynamic monitoring,clinical according to patients condition,as appropriate,to choose the appropriate monitoring method.
